Thrush, or scientifically candidiasis, is considered one of the most common fungal infections. Entire treatises have already been written about it, but the problem still remains relevant, since the fight against Candida fungi, which cause unpleasant symptoms, no matter where they are found, is quite difficult and requires a certain amount of patience and material costs. Today, there are already many different drugs from different manufacturers intended for the treatment of candidiasis, but in Soviet times, the choice of effective drugs was limited, and, nevertheless, doctors somehow solved the problem, using, for example, the antiseptic "Miramistin", known for more than 37 years, for thrush in adults and children.
A little about the disease itself
Thrush is a fungal disease, which in itself can hardly be considered a serious pathology. Fungi of the genus Candida are representatives of opportunistic microflora that live on human skin, so they are not considered dangerous if a person has a strong immune system.
A small number of microorganisms on the skin and mucous membranes is unlikely to cause an inflammatory process and the appearance of unpleasant symptoms indicating a disease:
- the appearance of a whitish, cheesy coating on the mucous membranes of the genitals in women and men or in the oral cavity (this localization of the fungus is often observed in children and people with weakened immunity),
- excruciating itching in the genital area, burning during urination, caused by irritation and inflammation of tissues affected by fungal infection,
- if the infection is localized in the genital area, an unpleasant odor may appear, the presence of fungus in the oral cavity is often accompanied by the appearance of a sour taste,
- The fungus causes inflammation of the mucous tissues and this can explain the pain in the genitals during sex and urination.
But when the fungal flora begins to increase, the first signals appear that something is wrong in the body. Doctors associate the increase in the number of fungal infections with a violation of the body's microflora, which provides human immunity. The death of beneficial microflora, in turn, can be caused by various reasons. These are chronic diseases, systemic or local use of antibiotics that do not have a special selectivity in their destructive effect on living microorganisms, the use of some antimicrobial pastes and mouthwashes or intimate hygiene products, the use of the same antifungal drugs.
Candidiasis is a contagious disease that can be easily acquired from a sexual partner with the same diagnosis. But even in this case, the symptoms of the disease can only appear in a person with a weak immune system that is unable to resist the proliferation of fungi and bacteria.
By the way, women can also get thrush as a result of improper care of the genitals. A manic desire for cleanliness throughout the body leads to the fact that the lady begins to regularly douche, believing that in this way she cleanses the vagina of possible pests. In fact, it washes out the beneficial microflora, which does not die, as with the action of antibiotics, but is simply removed from the body along with water. Changes in the acidity of the vagina lead to a decrease in local immunity, and fungi that accidentally got on the mucous membrane will now actively multiply.
Systemic antibiotics are potent drugs that can disrupt the intestinal microflora and the entire body, since they do not really understand who is a friend and who is an enemy. As for antifungal agents, the internal use of such drugs, like antibiotics, can equally cause vaginal or oral candidiasis, which confuses patients who, having gotten rid of the fungus in one place, can provoke its proliferation in another.
Both antibiotics and antifungal agents have a detrimental effect on the beneficial microflora of the body, so if there is an alternative treatment, it is better to try it first. The antiseptic "Miramistin" for thrush is just such an alternative, the effectiveness of which has been proven over time.
The popularity of Miramistin in the treatment of thrush is due to several factors:
- the safety of the antiseptic, which makes it possible to treat both adult patients and children,
- good effectiveness against fungal infections when applied locally (the drug does not actually penetrate into the blood, but still has a decent therapeutic effect),

- good anti-inflammatory effect, allowing to reduce the intensity of unpleasant symptoms quite quickly,
- absence of toxic effects on the body,
- availability (low price and the ability to purchase the drug without a doctor's prescription.
Although Miramistin is not considered an antibiotic, it has a fairly strong antimicrobial effect and is effective against many bacteria, viruses, and fungi. As an antiseptic, when used correctly, it is not prone to destroying beneficial microflora in the body, and even on the contrary, it helps to increase local immunity, which is considered a good preventative measure against relapses of the disease.
Another big plus of Miramistin is the wide range of application of this effective and universal antiseptic. That is, the remainder of the drug after treating thrush will not have to be thrown away, worrying about wasted money, because it can be used for other purposes, for example, treating scratches and ulcers on the skin, burn wounds, for the treatment of urethritis and the prevention of sexually transmitted infections. The drug will also help in the treatment of inflammatory and purulent-inflammatory diseases of the ears, throat, nose. Antiseptic "Miramistin" is an antimicrobial agent that can effectively fight many infections, including fungal ones. This made it possible to use a drug that does not belong to antifungal agents, even for the treatment of thrush. Moreover, the forms of release of the drug allow you to treat candidiasis on any part of the body.
"Miramistin" in the form of a spray is considered the most popular form of release of the drug for the treatment of fungal diseases, because it is equally successfully used for vaginal candidiasis and for thrush in the oral cavity in adults and children.
But in medical practice, according to the instructions for use, an antiseptic with a broad antibacterial effect is used for other purposes. The popularity of Miramistin in surgery and traumatology is explained by its ability to disinfect and promote rapid healing of infected wounds. The drug is also used for preventive purposes to prevent secondary infection in open wounds.
Antiseptic is also used to treat burn wounds in cases of thermal or chemical tissue damage of 1-3 degrees. If plastic surgery is required later, the medicine is used to disinfect and prepare the skin for the procedure.
In gynecology, Miramistin is used to treat postpartum wounds (cracks and tears in the vagina and perineum, incisions during cesarean section, etc.), which prevents their suppuration and infection. The antiseptic is also used to treat inflammatory pathologies of the internal and external genital organs in women caused by an infectious agent (bacteria, viruses and fungi), as well as to prevent infection of inflamed and irritated tissues.
Among other things, Miramistin is considered a good means of preventing sexually transmitted infections.
In gynecology and urology, Miramistin is used in the complex treatment of genital thrush in women and men. But therapists and dermatologists often prescribe an antiseptic for the treatment of skin and mucous membranes in case of candidiasis of the skin and oral cavity, mycosis of the feet and skin folds.
In ENT practice, antiseptics are used to treat the nose, throat, and ear canal in cases of tonsillitis, sinusitis, and otitis, both acute and chronic.
The drug has also found wide application in dentistry. It is used in the treatment of such pathologies as periodontitis (inflammation of the gums) and stomatitis (inflammation of the oral mucosa), as well as a disinfectant prophylactic agent after operations performed in the oral cavity, after tooth extraction, etc. The antiseptic is well suited for disinfecting removable dentures.

Strong antifungal agents are not the best option for treating a child, so pediatricians prefer to use more gentle medications that are active against fungal infections. Miramistin is one of these medications.
Sometimes diseases that are familiar to us have an atypical pathogen. For example, diseases of the respiratory and breathing systems do not always have a fungal or viral nature. Sometimes such diseases are caused by fungi, and then they say that the disease has a fungal nature, not calling it thrush, but writing in the medical record "otitis of fungal nature" or "candidal bronchitis", etc.
Many inflammatory diseases of the genitourinary system, requiring the use of antibacterial and antiseptic agents, can have a fungal nature, along with bacterial or viral ones. Therefore, "Miramistin" can be considered a universal medicine that helps with thrush, which is considered a fungal infection, and with fungal urethritis, and with bacterial vaginitis, and with many other diseases caused by fungi, bacteria or viruses.
A very important feature of the drug, despite the fact that it is a synthetic drug, is its non-toxicity and small number of side effects. The relative safety of the drug allows using Miramistin not only for the treatment of small children (and pediatricians prescribe the antiseptic even to infants), but also for thrush during pregnancy.
The drug is practically not absorbed into the blood, having only a local effect, therefore it does not pose a danger either to the woman herself or to the unborn child. This is very important, because the choice of drugs during pregnancy and breastfeeding is very limited, which complicates the treatment of such a tenacious pathology as thrush.
It would seem, why take risks and rush with treatment measures? If the disease is not so dangerous, then it would be possible to wait with its treatment until the child is born and grows up on mother's milk. In fact, if the mother has thrush, the baby already risks taking the disease upon itself when passing through the birth canal. And the young mother then no longer has to wonder where the newborn has a whitish coating in the mouth and in multiple folds of the skin.
If a mother does not want such a fate for her child, she will start treating thrush even before the baby is born, using such safe local remedies as Miramistin.
Well, it turns out that the antiseptic known for many years is simply a necessary medicine in any home medicine cabinet. A remedy that can be used if necessary by a future mother, a child, and an elderly person, without fear of the unpleasant consequences inherent in antifungal drugs.
Release form
"Miramistin" is an antiseptic agent that is widely used in various fields of medicine. It is clear that manufacturers have tried to make it as convenient as possible to use in any situation where the drug can have a therapeutic effect. To facilitate the use of the antiseptic in the affected area, which is not always easily accessible, pharmaceutical companies have developed several forms of the drug.
For the treatment of small wounds and diseases of the hearing organ, it is convenient to use the ointment "Miramistin", containing 0.5% of the active substance. The same ointment can be used to treat the external genitalia if irritation and wounds have appeared on them due to the influence of the fungus.
But for the treatment of thrush, regardless of the location of the infection, a 0.01% antiseptic solution is more suitable, which is available in bottles of different sizes. Bottles with a large dosage (100, 150 and 200 ml) are equipped with a special nozzle and act as a spray. The 50 and 100 ml bottles have a urological nozzle, which facilitates the introduction of the drug into the urethral cavity. The 500 ml bottle does not have nozzles and is used mainly in medical institutions.
As we can see, for thrush, Miramistin can be used in various forms of release, but the most convenient forms are still considered to be a solution and spray, the scope of application of which is much wider.

Pharmacodynamics
The drug "Miramistin" is a well-proven antiseptic developed in the 70s of the twentieth century. Like other antiseptics, it is considered an antimicrobial agent used for external treatment of wound surfaces and areas affected by pathogenic microorganisms, which include bacteria, viruses, fungi and some other representatives of harmful microflora.
Most antiseptics are designed to combat bacteria, which most often attack the damaged area. Similar to other antibacterial agents, Miramistin is capable of having a detrimental effect on many bacteria, both gram-positive cocci and other representatives of this group of microorganisms, and gram-negative microbes (Klebsiella, Escherichia coli and Pseudomonas aeruginosa). The antiseptic is used to combat aerobes and anaerobic representatives of bacterial flora, which are able to live and reproduce in places where air access is limited.
Among other things, the antiseptic helps fight microorganisms involved in the development of sexually transmitted diseases: trichomonas, chlamydia, treponema, the causative agent of gonorrhea, etc.
How does the drug affect the microbial cell, i.e. what is its pharmacodynamics? Interacting with the lipids of the bacterial cell membrane, it thus destroys the cell membrane, which becomes permeable to harmful substances. The enzymatic activity of the bacterial cell is also disrupted, i.e. the vital processes in it gradually fade away, which leads to the death of the microorganism.
The peculiarity of "Miramistin" is that it sometimes has a stronger local effect than antibiotics, because bacteria do not develop resistance to it. The antiseptic copes well both in the case of detection of one specific group of microorganisms, and in situations when entire microbial associations consisting of various types of bacteria, fungi, viruses, etc. are found in the affected area.
Some readers may wonder what a broad-spectrum antiseptic with an antimicrobial effect, similar in its action to antibiotics, has to do with thrush, which is considered a fungal infection. And as we know, antibiotics are incorrectly used in the treatment of fungal infections, the therapeutic effect from them will be temporary, and then the fungi will begin to multiply with even greater force at the site of the death of beneficial microflora. In this case, can Miramistin be used for thrush?
The antiseptic "Miramistin", unlike antibiotics and some other antiseptics, has a selective effect, i.e. it does not harm human cells and cells of beneficial microflora, and when used correctly does not disturb the balance of the body's microflora. And its high antifungal activity (it is active against ascomycetes, dermatophytes, yeast and yeast-like fungi, including the causative agents of thrush Candida albicans, Candida tropicales, Candida crusei) allows it to be used to treat various forms of candidiasis and some other fungal infections.
By the way, in some cases, Miramistin is more effective than antifungal agents, to which, like antibiotics, microorganisms have developed resistance, so treatment with such drugs is not successful.
"Miramistin" has also been noted to have antiviral activity, which manifests itself in an increase in local immunity. The drug is capable of fighting even viruses that are difficult to treat, such as the herpes virus, the human immunodeficiency virus (HIV), etc.
Pharmacokinetics
As for the pharmacokinetics of the drug, i.e. the processes that occur in the body with the participation of drug particles, the speed of reactions in which the active substance enters and some other parameters, there is no need to judge them. "Miramistin" is used for local treatment and acts mainly in superficial tissues (skin and mucous membranes). It does not penetrate into the deep layers and blood, so there is no need to talk about the ways and speed of its elimination from the body.
Dosing and administration
It was noted above that the antiseptic has very convenient forms of release, allowing it to be used even in hard-to-reach places, such as the urethra or vagina, where a fungal infection can be localized in men or women. For candidiasis of the genitals and oral cavity, the most convenient forms of the drug are considered to be a spray with a nebulizer and a solution with a special dropper nozzle. In principle, this is the same form of the drug, but different nozzles allow you to expand the scope of application of the drug.
For men, it is more convenient to use a drug in a bottle with a urological attachment to treat candidiasis on the penis. In a standing position, the solution is injected into the urethra using the attachment in a volume of 3 ml and the penis is wiped with it in the affected area to remove fungi and bacteria from the surface tissues. If the fungus is located on the surface of the urethra in its folds, then a spray can be used to treat the tissues affected by the disease, trying to get the medicine under the foreskin. The procedures should be carried out every day for 5-7 days.
Women are recommended to douche daily to treat vaginal candidiasis. How to douche with Miramistin? There are no difficulties in carrying out the procedure, since you can use the same solution with a nozzle, inserting it into the vagina and injecting about 8-10 ml of the solution per procedure.
It is recommended to douche in a lying position, lying on your back and spreading your legs to facilitate the insertion of the nozzle into the vagina. After the introduction of the medicine, it is advisable to lie down for 10-15 minutes so that the drug has time to act. After the procedure, you need to use a pad, since the remains of the solution may leak for some time and get on your underwear.
During douching, the solution is in the vagina for a very short time, which does not allow it to effectively affect the fungi. Douching is more of a cleansing procedure, and tampons with Miramistin are considered more effective in treating thrush. They also need to be placed every day for 1-2 weeks, leaving them in the vagina for a couple of hours.
The procedure is also carried out in a lying position. A tampon made of cotton wool and bandage is prepared in advance, soaked in an antiseptic solution and inserted into the vagina.
In pharmacies you can also buy suppositories with miramistin, which can be used instead of tampons, leaving them to act all night.
Depending on the severity of the lesion, the doctor may prescribe 1 to 4 procedures per day using Miramistin.
Use for children. In children, thrush develops mainly in the oral cavity or in skin folds. To treat the skin, you can use a spray or solution with a nozzle. If you need to irrigate the oral cavity with an antiseptic, it is more convenient to use the drug in the form of a spray.
The use of Miramistin for thrush in infants is very indicative. Oral candidiasis in newborns can be acquired from the mother or caused by an insufficiently formed immune system. In this case, a white cheesy coating can appear on the cheeks, tongue, pharynx, gums, tonsils and even the throat of the baby, gradually moving towards the esophagus. In this case, the affected area is very sensitive and painful, which can cause crying, refusal to eat, digestion and sleep disorders in the baby.
It is believed that Miramistin can be used in children from the age of 3. In fact, pediatricians successfully treat children even of infant age with it. But it is recommended to spray the medicine not into the child's throat, but onto the tongue or behind the cheek. The baby does not yet know how to hold his breath, so if the medicine gets into the respiratory tract, it can provoke bronchospasm.
For children under 6 years old, 1 spray of the medicine is enough. Children from 7 to 14 years old will need 2 presses on the spray valve. Teenagers need to spray the medicine into the oral cavity, making 3-4 presses. The frequency of use of the medicine can vary from 1 to 4 times a day.
Side effects Miramistin for thrush
When we talked about the safety of the drug "Miramistin" used for thrush or other diseases of fungal, bacterial or viral nature, we meant not only the good tolerability of the drug by patients of any age, but also the absence of any contraindications, with the exception of rare cases of hypersensitivity to the components of the drug. This is one of the few drugs, the use of which does not affect the functioning of the body in any way, especially since it is used locally.
But how about treating the oral cavity with candidiasis in this area, since some part of the drug must enter the gastrointestinal tract with saliva? This is what happens, but the whole point is that the active substance of the antiseptic is practically not absorbed in the stomach and intestines, so it cannot enter the blood.
The use of the drug rarely causes unpleasant symptoms. In rare cases, people with particularly sensitive skin may complain of a slight burning sensation that passes as quickly as it appears.
Burning at the site of application of the medicine is not a sufficient reason to cancel the drug. It is a different matter if other symptoms are added indicating increased sensitivity to the antiseptic. If tissue hyperemia, skin itching and severe burning, increased dryness of the skin or mucous membranes appear, it is better to refuse to use the solution or ointment.
Overdose
As for an overdose of Miramistin, it is excluded when used externally and is unlikely when the solution is accidentally swallowed.
Interactions with other drugs
When using various medications, manufacturers recommend paying attention to such a point in the instructions for the drug as drug interaction or interaction with other drugs. In the case of Miramistin, no negative interactions with other drugs were noted. However, it was noted that the antiseptic increases the sensitivity of bacteria to antibiotics, which only enhances the effect of treatment with the latter. This is very important, since the problem of antibiotic resistance has become the scourge of modern medicine.
Storage conditions
The storage conditions for the medicine are extremely simple. It perfectly retains its beneficial properties at room temperature, but it is better to ensure that the thermometer readings in the room where the antiseptic is stored do not exceed 25 degrees. Although Miramistin is generally considered a safe drug, it is still a drug, so manufacturers recommend keeping it away from children.
Shelf life
The drug in solution form can be used for 3 years from the date of manufacture. However, in order for the treatment to provide the expected antiseptic and antifungal effect, the expiration date of the drug must be monitored.

Popular analogues of "Miramistin"
The pharmaceutical industry today produces a great variety of different antiseptics that are used for local treatment of diseased or damaged areas of the body. Some of these drugs have more or less pronounced antifungal activity, which puts them on par with Miramistin. But when it comes to treating thrush on the mucous membranes and genitals, it turns out that not all antiseptics are ideal for treating such an unusual (generally harmless, but very unpleasant) disease.
For example, a noticeable antifungal effect is characteristic of coloring antiseptics: iodine, brilliant green, fucorcin solution, the drug "Iodinol", potassium permanganate crystals (manganese). No one forbids using these drugs to treat thrush, but you need to understand that the places of application in this case will be colored in different colors: yellow-brown, green, pink. Moreover, some of the antiseptics may contain alcohol and cause a burn if used incorrectly (they must be diluted with water).
Let's see how you can use coloring antiseptics like Miramistin for thrush.
Alcohol solution of iodine. It should never be applied to mucous membranes in its pure form. This will not enhance the effect of the treatment, but will most likely cause severe burns of the mucous membranes.
For sitz baths for thrush, add 1 tbsp of iodine (or 1 tsp of iodine and soda) to 1 liter of water. The procedure is carried out for 20 minutes.
For douching, iodine is used in combination with water, salt and soda (15 g of iodine, 1 liter of water, 30 g of salt and 15 g of soda). Douching is done twice a day for 5 days.
A solution of iodine and water is a weak antifungal antiseptic by itself, it rather relieves inflammation caused by a fungal infection. Another matter is soda, which actually effectively fights fungus in combination with iodine.
It would seem that iodine provides the opportunity for a fairly effective and inexpensive treatment of candidiasis, but those who have endocrine system disorders should be careful with it. And the proportions must be strictly observed so as not to burn the skin and mucous membranes.
Brilliant green solution (zelenka). This preparation for the treatment of thrush is also not used in its pure form. But it should not be added to water, but to a composition that is half three percent hydrogen peroxide, and the second part is boiled or distilled water. No more than five drops of brilliant green are added to the composition and used for daily douching for 1 week.
It is believed that such treatment of thrush in women quickly and cheaply removes unpleasant symptoms: itching, odor, discharge. But you need to understand that brilliant green is a strong drying agent that can lead to excessive dryness of the mucous membranes, on which cracks and inflammation will appear. In addition, you need to be prepared for the skin and mucous membranes to acquire an unnatural color for some time. In addition, the treatment is often accompanied by a pronounced burning sensation at the site of application.
Fukortsin solution. We are used to considering this drug as a remedy for chickenpox, which is used to lubricate rashes on the body of children. But few people know about the antifungal properties of this antiseptic.
The drug is used undiluted. The solution should be applied to the affected areas several times a day, which is not very convenient for treating vaginal candidiasis. A cotton swab soaked in the drug can be used to treat the affected areas in the oral cavity, external genitalia, and skin, but it will be difficult to inject the medicine into the body.
But again, a budget effective antifungal antiseptic turns out to be not such a safe medicine. Both fuchsin and boric acid in the composition of the drug are considered toxic substances that can cause poisoning of the body with prolonged use. In addition, frequent use can provoke a burn or swelling of the mucous membranes, the development of dermatitis, not to mention the fact that for 3-4 days you will have to observe the staining of tissues in a bright crimson color.
The drug "Iodinol". It is an aqueous solution of iodine compounds, which inhibits the reproduction of fungal infections and can also be used to treat thrush.
For douching and treatment of the external genitalia, prepare a solution of equal parts of boiled water and a medicinal product. Douching is carried out daily for a week.
Men can use this composition for compresses, applications and regular wiping of the penis for thrush.
"Iodinol" can also be used to treat oral candidiasis by dissolving the medicine in water and using it to rinse the mouth or moisten the mucous membranes in adults and children.
The diluted preparation does not cause severe drying or discomfort when applied to the mucous membranes. But again, it is not suitable for treating people with endocrine disorders and often causes allergic reactions.
Potassium permanganate. An antiseptic in the form of small dark crystals, sold in glass bottles. Used diluted with water. The product is caustic and can cause mucous membrane burns if the dosage is exceeded.
For douching and washing the genitals with candidiasis, you need to use a weak solution of "potassium permanganate". There is no need to talk about a specific dosage.
