Today, one of the central problems of gynecology is various modifications and inflammations of the endometrium. For various reasons, inflammatory, infectious, hyperplastic processes can develop in it. Formation of polyps, cysts, tumors is a common occurrence. At present, treatment of endometrial polyps remains quite relevant. Numerous discussions are underway among scientific researchers and practicing specialists aimed at finding the best way to solve the problem. Some are inclined to conservative methods, using folk remedies, while others believe that the best option for solving the problem is a radical method.

What to do if you have an endometrial polyp?

If polyps are present, you should see a gynecologist regularly. If it is possible to wait with surgical treatment methods, the doctor will prescribe conservative treatment. When a polyp is detected, diagnostics are carried out immediately, since without a diagnosis it is impossible to choose the right treatment.

First of all, it is necessary to differentiate a malignant tumor from a benign one. The tactics of further treatment depend on this. For this, a biopsy is performed. During hysteroscopy, using special instruments, a tissue sample is taken for further histological examination. After collection, under sterile conditions, the material is sown on special nutrient media intended for culturing tissue samples. They are placed in specially created conditions that are optimal for this culture. Incubated for a certain period of time.

After the material has grown, it is subjected to further research and interpretation. Based on the shape, size, location of the cells, nature and direction of growth, it is possible to determine whether the tumor is benign or malignant. The further plan of action depends on this. If the tumor is malignant, or there is a risk of the polyp degenerating into a cancerous tumor, the tumor is surgically removed as soon as possible. If the tumor is benign, conservative treatment can be used.

A polyp is always a benign neoplasm, but it can easily be confused with a cancerous tumor. Therefore, the only reliable method confirming the structure of the neoplasm is a histological examination. In addition, if there are atypical cells in the polyp, or it is damaged, there is a risk of its transformation into cancer. A polyp is an overgrowth of endometrial tissue that occurs for various reasons. The endometrium becomes inflamed and increases in size. Gradually, a small area of hypertrophied mucous membrane is localized, forming a polyp.

There are many different options for dealing with a polyp. It can be removed surgically. There are many different methods: open abdominal surgery, hysteroresectoscopy, endoscopic removal.

Abdominal surgery is now used extremely rarely, since its use is not justified. It carries many risks associated with the fact that a complete dissection of the abdominal wall, uterus, after which the mass polyp is removed. Such an operation has a long recovery period, since an open wound with a fairly large surface area is formed. At the same time, the risk of infection, postoperative complications, and bleeding increases significantly. The operation requires serious preparation and has a number of contraindications. Today, it is considered an outdated method. Nevertheless, in many hospitals where there is no high-tech equipment and highly qualified specialists, this method continues to save the lives of many women.

Endoscopic surgery is more commonly used, in which no abdominal incision is made. In this case, using special equipment (an endoscope), several punctures are made, through which an instrument with a camera is inserted, allowing the condition of the endometrium to be assessed, the polyp to be examined, and the optimal method of its removal to be identified. The image can be visualized on the screen, which significantly simplifies the surgeon's work. Then, using surgical instruments that are inserted through the opening of another endoscope, the neoplasm is removed. The instruments are removed, after which the openings are usually sealed with adhesive tape (sutures are rarely required).

This method has a number of advantages that make it more popular and frequently used. The risk of bleeding, postoperative complications, and infection is practically eliminated. Recovery is fast, and postoperative pain is insignificant.

The hysteroscopy method is the most gentle. It involves the introduction of a special instrument – a hysteroscope. It is inserted through the cervix, naturally. After that, an operation is performed to remove the polyp. The instrument is equipped with a camera that allows you to visualize the image. Damage is minimal, risks are also minimized. Recovery is much faster.

In addition to surgical methods, physiotherapeutic and medicinal ones are also used. Laser treatment has proven itself well. With this method, the affected areas are cut out, while the undamaged ones remain intact. Complications are rare, healing occurs within 5-6 weeks.

The method of choice is cryodestruction, during which the affected areas are cauterized with liquid nitrogen. Nitrogen crystallizes water and leads to the destruction of damaged areas. It is used mainly for damaged polyps and bleeding. This method can only be used for patients who have given birth, since it leaves scars on the uterus. It is painful and has a long rehabilitation period.

If it is necessary to amputate damaged areas, radio wave methods and the conization method are used. These methods are usually used if the process of malignant transformation of the polyp has begun.

Treatment of endometrial polyps without surgery

Today, treatment of endometrial polyps is perhaps the main problem of gynecology. The frequency of occurrence of this pathology increases every year. The main reason for the formation of polyps is the disruption of the normal hormonal background, in which estrogen predominates and a sharp lack of progesterone is revealed. When polyps are formed, excessive compaction of the mucous membrane of the inner layer of the uterus occurs, as a result of which the compacted area gradually becomes isolated. Separate blood vessels are formed, the polyp acquires its own innervation. This contributes to its further growth.

In some cases, polyps are treated with traditional methods. The main method is hormone therapy. If conservative therapy is ineffective, surgical methods are prescribed. Treatment is required because polyps pose a serious threat to a woman's health. Polyps can greatly increase in size. The membrane can swell so much that it gradually extends beyond the uterus and even fills the vagina.

Moreover, a polyp in the endometrium is capable of intensively dividing and forming multiple polyps that cover the entire uterine cavity. Gradually, such polyps degenerate into malignant neoplasms, acquiring the ability to divide uncontrollably.

Drug therapy involves the use of hormonal and non-hormonal agents that promote the resorption of the polyp and the reduction of hypertrophic areas. Symptomatic therapy is also used: painkillers, anti-inflammatory drugs, if necessary - antipyretics, antispasmodics. Traditional methods, homeopathic remedies and herbal medicine are widely used.

Medicines

Medicines for polyps should be taken with caution, observing safety precautions. It is imperative to consult a doctor before using any, even the most harmless, remedy. Self-medication can be dangerous due to its consequences and side effects, the most dangerous of which is the transformation into a cancerous tumor.

With the development of an infectious and inflammatory process and progression of the disease, streptomycin sulfate is prescribed at 1 gram per day. This drug has antibacterial properties, quickly eliminates inflammation, reduces endometrial hypertrophy. It is necessary to strictly adhere to the prescribed dosage and treatment regimen, undergo a full course of treatment, otherwise the infection may develop resistance. In addition, progression of the disease is possible.

In any hypertrophic process, it is recommended to take medications that help reduce the inflammatory process, resolve compactions, normalize vascular permeability and tissue metabolism. It is recommended to take acetylsalicylic acid 2-3 grams per day, voltaren 0.1-1.5 grams per day.

If severe pain occurs, take painkillers. It is recommended to take ketanol 50 grams per day. No-shpa also works well, which additionally relieves spasm.

Duphaston for endometrial polyps

Duphaston is one of the most effective drugs traditionally used to treat polyps. It belongs to the group of gestagens (hormonal drug). It is a synthetic analogue of the female hormone, which normalizes the structural and functional state of the reproductive organs. The drug should be prescribed only by a doctor, having previously checked the hormonal status. Self-medication can end in a deterioration of the condition and excessive growth of the polyp, its malignant degeneration.

The drug also normalizes the menstrual cycle and helps maintain pregnancy. The main indication is the formation of polyps due to a lack of progesterone. It is mainly used to treat glandular and fibrous polyps. It is a good alternative to surgical treatment methods and helps resolve polyps. It is often prescribed to nulliparous girls and when surgery is not possible.

It is taken in the second phase of the menstrual cycle, according to an individually selected scheme. The minimum course of treatment is 3 months. Sometimes the treatment can drag on for several years. The duration of therapy and its effectiveness are monitored by hormone tests.

The drug has virtually no side effects. There is not even the development of secondary sexual characteristics (excessive hair growth), which are often observed when using other hormonal agents. It is available in tablet form, so the drug is prescribed orally. The highest level of hormone concentration can be achieved within a few hours after taking the drug. The drug is compatible with almost any drug.

The main advantage of this drug is that it does not interfere with conception. It can be used even during pregnancy. The drug is contraindicated in Rotor syndrome, Dubin-Johnson. It is also not used in case of individual intolerance to individual components of the drug. People with kidney disease should take it with caution. Side effects include headaches and migraines. Less common are itching, burning, and skin rashes.

Physiotherapy treatment

Physiotherapy can contribute to the treatment of polyps. They use light beams of different wavelengths. Thus, ultrasound can penetrate deep into tissues, exerting an anti-inflammatory, stimulating effect on them. Infrared light can normalize microflora, eliminate congestion, and, accordingly, reduce tissue swelling, hyperemia, and hyperplastic processes. Using a laser, you can localize areas of necrosis, remove affected parts of the endometrium, while leaving intact tissues intact. Using electrical procedures, you can stimulate certain areas of the uterus, endometrium, causing activation of the cells of the mucous membrane, the immune system.

Electrophoresis and phonophoresis allow the most accurate introduction of the necessary drug directly into the endometrial tissue. At the same time, the daily dosage is significantly reduced. The drug penetrates through the skin or mucous membranes under the influence of microcurrents. At the same time, systemic effects on the body are excluded, due to which the likelihood of side effects and overdose is significantly reduced.
