Causes and pathogenesis of gangrene of the penis and scrotum. It is believed that gangrene of the penis and scrotum is caused by Staphylococcus aureus, Streptococcus, and less often by Proteus. In the pathogenesis of the disease, an important role is played by sensitization to pathogens and their decay products, the development of allergies, the involvement of skin vessels, the development of ischemia and necrosis.
Symptoms of gangrene of the penis and scrotum. The disease often develops in men unexpectedly, against the background of complete health, without any reason. Swelling of the penis and scrotum, fever (38-39 ° C), malaise are noted. After a few days, superficial necrosis develops, capturing the entire anterior part of the scrotum and penis, causing tissue destruction. The testicles are never involved in the process due to the elasticity of the skin of the scrotum and the fibrous membranes that protect them. In most patients, the intensity of pain is expressed in a weak or moderate degree.
When the disease develops in women, the pathological process is often localized on the labia minora or in the clitoral area, moving to the perineum, less often to the pubis and thighs.
Leukocytosis and increased ESR are observed.
Differential diagnosis. Gangrene of the penis and scrotum should be distinguished from cancer, complicated form of hard chancre, chancrinform pyoderma.
Treatment of gangrene of the penis and scrotum. It is recommended to determine the sensitivity of microflora to antibiotics, prescribe broad-spectrum antibiotics in shock doses.
General tonic and vitamin preparations are used, externally - aniline dyes, ointments and creams containing steroids and antibiotics.
