Brittle nails are not a single diagnosis, but a clinical syndrome in which the nail plate becomes thin, less elastic, and begins to crack, crumble, flake, or break along its length and free edge. In dermatology, two main phenotypes are most commonly distinguished: onychoschisis, which is transverse lamination of the free edge, and onychorrhexis, which is longitudinal grooves and cracks. These variants can exist separately, but in many people they occur together. [1]

From a practical standpoint, brittle nails develop when the bonds between the layers of the nail plate are disrupted or the normal formation of the nail in the matrix, that is, in the growth zone, is altered. The former situation is more often associated with external influences, such as water, detergents, solvents, and mechanical trauma. The latter more often suggests dermatological, systemic, or medicinal causes. [2]

This condition is very common. A review of brittle nails indicates that the problem affects approximately 20% of the population, with a higher prevalence in women and older age groups. Age-related changes in the nail plate, decreased lipid levels, dry air, and the accumulation of microtrauma also play a role. [3]

Nails have one important characteristic: they grow slowly, so any damaging factors don't act suddenly, but cumulatively. The average growth rate of fingernails is about 3.47 millimeters per month, while for toenails it's about 1.62 millimeters per month. This means that even with proper treatment, noticeable results usually appear not within a few days, but rather over weeks and months. [4]

Most often, a person with brittle nails does not have a serious underlying medical condition. The most common mechanism is repeated wetting and subsequent drying of the nail plate, especially when combined with detergents, frequent manicures, acetone-containing products, polishing, and the habit of picking or scraping off the nail polish. However, the problem cannot be entirely attributed to nail care alone, as similar symptoms can be caused by fungal infections, psoriasis, eczema, lichen planus, iron deficiency, thyroid disease, and certain medications. The main causes of brittle nails

The most common cause is external damage to the nail plate. Repeated cycles of water and moisture evaporation are particularly damaging to nails: first, the horny layers swell, then contract, and the bonds between them gradually weaken. This mechanism is well documented for both household contact with water and work involving frequent hand washing, cleaning, kitchen work, and humid environments. [7]

Mechanical and cosmetic factors rank second. Aggressive polishing, metal files, rough cuticle removal, peeling of gel polish, frequent use of acetone-containing products, prolonged wear of artificial nail polishes, and contact with acrylates can not only increase brittleness but also cause allergic contact dermatitis with additional nail dystrophy. Modern reviews of home nail care products confirm that allergic and irritant dermatitis is one of the main complications of such procedures. [8]

A separate, large group is fungal nail infections. They don't always begin with the classic thickening and yellowing. Sometimes the first noticeable sign for the patient is brittleness, crumbling, and roughness. But an important detail is that not every brittle nail is infected with fungus: visually, a fungal infection can resemble psoriasis, traumatic dystrophy, eczema, and other conditions. This is why modern reviews recommend laboratory confirmation of the diagnosis. [9]

Dermatological diseases also frequently involve the nails. Psoriasis can cause pitting, discoloration, subungual hyperkeratosis, peeling, and brittleness. Lichen planus often causes thinning, longitudinal cracks, and more severe matrix damage. Hand eczema and allergic contact dermatitis can be accompanied by inflammation of the periungual tissues, cuticle loss, brittleness, and secondary dystrophy. [10]

Systemic causes are less common, but they are important to avoid. Iron deficiency can lead to thinning, brittleness, and spoon-shaped nails. Thyroid disease can also alter the condition of the nails: hypothyroidism has been associated with slow growth, brittleness, thickening, and dryness, while thyrotoxicosis can also cause brittleness and onycholysis. Nail changes alone do not confirm the diagnosis, but when combined with other symptoms, they warrant examination. [11]

Finally, brittleness may be related to medications. Nail changes associated with systemic retinoids have been described in the literature, and a recent 2024 study found that isotretinoin increases the risk of nail changes, primarily onychoschizia, and that these changes are usually reversible after treatment. Therefore, it is always important to ask not only about medical conditions and care, but also about medications being taken. [12]

When brittle nails require an in-person assessment

In many cases, brittle nails are an unpleasant but not dangerous problem. However, there are situations that require not only care but also a full medical examination. The first such situation is a rapid change in the appearance of one or more nails without an obvious external cause, especially if thickening, significant discoloration, peeling, pain, or inflammation of the periungual tissues appear. Such signs increase the likelihood of infection, inflammatory dermatitis, or tumor of the nail apparatus. [14]

The second group of warning signs is associated with systemic symptoms. If brittle nails are accompanied by pallor, weakness, hair loss, dry skin, shortness of breath, cold intolerance, constipation, or other general complaints, the likelihood of iron deficiency or thyroid disease increases. Nails are not the primary criterion here, but they become part of the overall clinical picture. [15]

The third important situation is suspected fungal infection. If a nail has begun to crumble, thicken, change color, or become separated from the nail bed, or if multiple toenails are affected, especially if there is a concurrent foot fungus, self-treatment with nail strengtheners and oil serums is unlikely to help. A diagnostic investigation is necessary, as confirmed onychomycosis is treated differently than typical dry, brittle nails. [16]

Another reason to see a doctor is if proper care fails to produce results. If nails continue to deteriorate within 2-3 months after eliminating the abrasive factors, regularly moisturizing, and following a gentle regimen, this is an argument against a purely household cause. Considering the slow growth of nails, this period is sufficiently informative to assess the progress of the process. [17]

Finally, an in-person evaluation is necessary in the presence of severe pain, purulent discharge, bleeding, nail destruction, a dark streak or spot that changes shape and width, as well as unilateral, persistent damage to one nail without a clear explanation. These symptoms go beyond simple brittleness and require separate diagnostics. [18]

Diagnostics: What Really Matters

The basis of diagnosis is a detailed interview and examination. The doctor evaluates the predominant type of brittleness, how many nails are affected, whether the hands, feet, or both are affected, the appearance of the skin on the hands and feet, and whether there are signs of inflammation, psoriasis, eczema, athlete's foot, or allergies. At the same time, the patient's occupation, grooming habits, frequency of contact with water, use of gel, acrylic, and solvents, and a list of medications are clarified. Even at this stage, it often becomes clear where to look for the cause. [20]

If the diagnosis is consistent with a fungal infection, the next step is laboratory confirmation. Current reviews and recommendations recommend direct microscopy after treatment with potassium hydroxide, culture, histological examination of the nail plate, and, if available, molecular methods as the primary methods. Direct microscopy is convenient as a quick and specific initial test, while culture or additional methods help to clarify the pathogen and resolve controversial cases. [21]

If systemic causes are suspected, the examination should be more focused rather than exhaustive. If there are signs of iron deficiency, a complete blood count and ferritin are typically assessed. If there are symptoms of thyroid dysfunction, thyroid-stimulating hormone and, if necessary, thyroid hormones are assessed. The idea here is simple: don't look for "every trace element in sight," but rather test for the causes that are truly supported by the clinical picture. [22]

Dermoscopy of the nails and nail bed can be helpful in complex cases, particularly when it's necessary to differentiate between fungal infections, traumatic dystrophy, and inflammatory nail diseases. However, this method is a complement to, not a substitute for, laboratory confirmation of fungal infection. This is important because the visual similarities between nail diseases are very high. An overly broad examination without a clinical basis is often useless. For most patients with typical everyday fragility, it is sufficient to exclude obvious local triggers, assess the risk of fungal infection, and, if systemic complaints are present, perform a brief, targeted screening. This approach is consistent with the modern principle of rational diagnostics: not to miss important findings, but also not to overload the patient with random tests. [24]

Treatment: What works best?

If brittleness isn't caused by a specific nail disease, the mainstay of treatment isn't a pill, but eliminating the damaging factors. The most important measures are to reduce frequent, prolonged contact with water and household chemicals, wear protective gloves when working with wet nails, reduce nail buffing and trauma, avoid removing nail polish, and adopt gentle nail care. This approach best aligns with the mechanism of common onychoschizia. [26]

The second key element is regular moisturizing of the nail plate and the skin around it. Reviews of brittle nails and nail care products most often mention products with moisturizing and softening ingredients, including urea, glycerin, lactic acid, lanolin, and occlusive bases. The goal of such care is to reduce dryness and improve the flexibility of the nail plate. Consistency is more important than a specific brand. [27]

If a fungal infection is confirmed, treatment is no longer for brittle nails, but for onychomycosis. The approach depends on the prevalence, thickness of the lesion, number of nails, comorbidities, and the risk of drug interactions. But the basic principle remains the same: first confirm, then treat. This reduces the risk of unnecessary treatment and errors when psoriasis or traumatic dystrophy are mistakenly treated as a fungal infection. [28]

If brittle nails are associated with eczema, psoriasis, contact dermatitis, or another inflammatory condition, the underlying condition needs to be addressed. In such cases, strengthening nail polish alone rarely solves the problem, as the source of the damage is inflammation of the nail system or surrounding skin. Therefore, the treatment plan typically combines nail care with treatment of the underlying dermatitis. [29]

If iron deficiency or thyroid pathology is detected, the key step is correcting the underlying cause. The nail plate is not immediately "repaired" after normalization of test results, as a healthier nail must grow back. Biotin has been studied for brittle nails, and some small, older studies have shown improvements in nail thickness or hardness, but modern reviews emphasize that the high-quality evidence base for its routine use remains weak. Additionally, one must be mindful of the risk of distorting laboratory tests at high doses of biotin, including troponin and certain hormone tests. Prevention and daily care

The best prevention begins with simple household rules. When doing any wet work, cleaning, washing dishes, or coming into contact with chemicals, it's best to protect your nails with gloves. It's important to ensure your hands are dry before putting them on, and that wearing gloves for too long without a break doesn't lead to constant maceration. Cotton liners under protective gloves are especially helpful for those with sensitive skin. [33]

It's best to keep nails short or medium length and file them gently, avoiding sawing the nail in all directions. A shorter length reduces the leverage on the free edge and reduces the risk of further cracks and splitting. The key here is not so much achieving a "perfect shape" as reducing ongoing microtrauma. A separate preventative measure is a sensible approach to decorative coatings. The problem is usually not the manicure itself, but the frequency of aggressive procedures: deep buffing, rough material removal, frequent use of harsh solvents, and prolonged exposure to potential allergens. Modern reviews of at-home nail care products remind us that it is improper techniques and acrylates that most often cause complications. [35]

Diet also matters, but not in the simplistic sense of "my nails lack vitamins." Nails do depend on overall protein intake and iron stores, but in the absence of a deficiency, uncontrolled supplementation rarely yields a convincing additional effect. It's far wiser to first evaluate your diet and, if necessary, confirm a deficiency with laboratory tests. [36]

The most realistic prognosis for normal nail brittleness is good, but slow. If damaging factors are eliminated and care is improved, the first improvements are often visible within 4-8 weeks, with more complete results later, as the new nail plate grows.
