Os ilium - the iliac bone is considered to be one of the largest, large and strong bones of the human skeleton. In fact, there are two of them - the right and left iliac bones, they are localized at the top of the pelvic bone. Both bones belong to the paired group and have the same dimensions. In structure they also do not differ from each other, each consists of a wing and a body. The ileal cyst can form at any site, but most often it is diagnosed in the wing, since it is more susceptible to strain, connecting the ear with the sacrum and pelvic bone.

Solitary and aneurysmal cysts can develop in the ilium, doctors note that CCM is more often detected in children under 15-16 years old, aneurysmal ones in adult patients. Patients of the older generation may also complain of signs similar to those of the bone cyst, however this symptomatology is more likely to refer to other specific osteopathology.

Symptoms of the ileal cyst

The cyst of the ilium can develop latent for several years, occasionally expressed as lumbing pain in the pelvic region. The most typical symptom of a cyst is a fracture of the bone, which can happen both spontaneously, and from a small impact, a fall.

Symptoms of fracture: 

  • Sharp pain in the pelvic region, in the buttock zone.
  • Puffiness in the area of injury.
  • The pain is aggravated by the motion of the foot.
  • Decreased leg function, decreased movement volume.
  • Possible internal hemorrhage and bruising in the upper thigh area.
  • In children, the abdominal muscles tense up as a compensatory response.

Treatment of the ileal cyst

The cyst os ilium in 60-70% is treated surgically, conservative treatment is not only inconclusive, but it can also carry the risk of active cyst growth and pathological fracture. A large cyst is to be removed, resection is performed in an accessible way, filling the excised part of the tissue with a bone allotransplant. The transplant is laid vertically to strengthen not only the bone, but also to prevent protrusion of the femoral head. The wound surface is sutured layer by layer, after the healing of the incision the seam is practically invisible. Remodeling of the ilium is slow, but with modern restorative osteomaterials, tissue repair has a favorable outcome.

If the fracture occurred, in small patients and adolescents, in most cases a decrease in the size of the tumor and its disappearance is noted. In such situations, the following actions are shown: 

  • The patient should comply with bed rest during the entire treatment, and immediately after the fracture, try to lie down.
  • The lying position is under the knees of a small cushion.
  • An anesthetic and immobilization (a tire or gypsum) of the foot for a month is performed in the hospital.
  • After 4-5 weeks, the plaster is removed, a control examination is performed showing the condition of the cyst and bone tissue.
  • If the cyst is asleep, physiotherapy, massage and exercise therapy are prescribed.

Full recovery of normal leg functions is possible after 3-6 months after the start of treatment.
