For some reason, it is generally accepted that discharge from the reproductive organs of different colors and textures is the prerogative of women. And when men notice something similar, panic immediately begins. There are cases when such concern is justified, everything depends on the quality and quantity of the secreted secretion. But white or transparent discharge in men should not always be associated with some pathology. There are several types of physiologically conditioned discharge, and there is no reason to sow panic here.
Causes transparent discharge in men
Let's try to figure out in which situations normal hygiene procedures are sufficient, since the discharge is physiologically determined, and when it is worth rushing to see a male doctor.
What kind of discharge is considered normal?
There are many glands in our body. Each gland secretes a secretion that is characteristic only for it. The sex glands in humans secrete their secretion when sexual arousal occurs. In women, the highest degree of arousal is manifested by an increase in the volume of transparent mucous secretions that act as a lubricant during the act of copulation, in the stronger sex - by the release of semen, which in biology is called sperm.
In fact, sperm is a two-component composition consisting of a liquid part, which is a secretion of the sex glands, and spermatozoa. The latter enter the secretion of the sex glands not constantly, but only during ejaculation, which is the culmination of coitus or the act of masturbation.
Identical discharge may occur at night in men who have not experienced sexual release for a long time for various reasons, or in male adolescents during puberty. This involuntary ejaculation (wet dreams) is also considered a normal variant, even if it occurs 2-3 times a week.
Sometimes on the head of the penis and under the foreskin of men and boys, you can see a cluster of whitish oily ointment-like substance with a sharp odor. This is spegma, which is a secretion of the sebaceous glands mixed with moisture and dead epithelial cells. There is nothing scary in its secretion. There is a danger when a man neglects hygiene procedures and spegma becomes a breeding ground for bacteria, causing inflammatory processes in the area of the folds of the foreskin and the head of the penis (balanitis, balanoposthitis).
And finally, transparent discharge in men, which is considered normal. We are talking about the secretion of the urethral glands, which they secrete against the background of sexual arousal. This condition is called libidinal urethrorrhea, and it is considered normal regardless of the amount of secreted secretion. The volume of secretions, in turn, depends on the characteristics of the male body and the duration of sexual abstinence.
Sometimes such discharge is observed during straining, for example, during bowel movements. And there is nothing dangerous about this either.
In some cases, spermatozoa are found in the urethral secretion, albeit in small quantities. And yet they remain active enough to fertilize an egg if the almost transparent secretion enters the woman's body.
Pathogenesis
In addition to physiological discharge from the penis, men may have discharge that is not typical for a healthy body. Moreover, it is very difficult to differentiate physiological discharge from pathological discharge without special studies and studying the patient's complaints.
It is difficult to talk about any specific characteristics of the secreted secretion that would indicate its pathogenicity, except perhaps pus and an unpleasant odor. But not only purulent discharge, but also mucopurulent and even transparent mucous, which are not much different from urethremia, can indicate diseases in men.
Transparent discharge, however, like white, yellow, greenish, bloody and others, can be more liquid or thick consistency. The volume of discharge can also vary, which does not always characterize the pathology with which they are associated. Most often, abundant discharge simply indicates severe inflammation covering a large area of the skin of the genital organ.
In some cases, discharge occurs spontaneously in the morning or evening hours. It can be either constant or periodic. Sometimes, the secretion is provoked by alcohol consumption. In other cases, transparent discharge is observed after the act of urination.
The causes of transparent discharge in men can be different. In other words, one and the same symptom can characterize different diseases. Very often, discharge from the penis is caused by inflammatory processes in the organ associated with various infectious factors (these can be pathogenic bacteria that have entered the body from the outside, or native opportunistic microflora).
Most of the discharge from the urethra is associated with venereal diseases or as they are also called STDs (sexually transmitted diseases), which, according to statistics, in 80 percent of cases occur without the appearance of specific symptoms. That is, discharge and other symptoms may appear already at an advanced stage of the disease.
It is important to understand that the causative agent of inflammatory and venereal diseases can be either one type of pathogenic microflora or a whole bacterial bouquet. For example, a combined infection is often observed with STDs (Trichomonas and Chlamydia, gonococci, mycoplasma, ureaplasma, etc.). It is clear that the symptoms of diseases will change in accordance with the infection present. Accordingly, it is not necessary to talk about specific characteristics of discharge for each disease.
But inflammatory pathologies of the penis are not always associated with infection (bacteria, viruses, fungi). Risk factors for such pathologies can be considered organ injuries and surgical interventions (in this case, inflammatory discharge is considered an unpleasant consequence or complication of the injury or surgery). In some cases, discharge from the genitals is associated with nervous disorders, in others, the cause may be oncological diseases.
Symptoms
If a man's discharge does not fit the description of physiological, and is also accompanied by a certain discomfort in the penis area (burning, itching, pain, unpleasant odor), this can be considered the first signs of an incipient disease. Yes, most often it is the discharge that appears first, but men do not always pay attention to it if it is actually colorless. Another matter is purulent, yellow or greenish discharge, which clearly cannot be a normal variant.
However, transparent discharge in men is not always physiological, so you need to treat their appearance with a certain degree of caution, observing any changes in their characteristics. If suspicious symptoms appear or your health worsens, you should definitely visit a doctor for a consultation with a subsequent examination. And for preventive purposes, it would not be superfluous to clarify how physiological the existing symptoms are.
So, transparent discharge from the urethra in men can be normal, ensuring the natural functions of the reproductive system, or associated with various diseases. Thus, transparent discharge without a foul odor can manifest itself in men as physiological urethremia. But a similar symptom can also mark an inflammatory process inside the genital organ. After all, any inflammation is accompanied by the release of exudate, and the more severe the process, the more abundant the discharge.
Most often, we are talking about such an inflammatory pathology as urethritis. With inflammation of the urethra, a common symptom is the release of a mucus-like transparent liquid mixed with pus. The composition of such secretions includes inflammatory exudate, leukocytes and mucus actively secreted by the internal walls of the urethral canal. But inflammation can be caused by various factors. These can be infectious agents and causes not associated with bacterial, viral or fungal infection. The latter include: injuries, operations, allergic reactions, mechanical irritation when stones and crystals are released with urine. External inflammation of the skin of the penis can also be a consequence of its irritation by chemicals.
Infectious inflammation can be caused by bacteria present on our skin and in the body. In most cases, these are streptococci and staphylococci, but the contribution of E. coli, Proteus and other pathogens to the development of inflammatory processes should not be underestimated. Moreover, the inflammatory process can be localized not only in the urethra, but also in the bladder, kidneys, and prostate. Depending on the pathogen, localization and degree of the inflammatory process, discharge from the penis (which in men is also the urethra) can be transparent or translucent with an admixture of pus.
Non-gonorrhoeal urethritis may also be accompanied by other symptoms. Men may initially experience very scanty transparent discharge, which later becomes mucopurulent and more abundant, and pain in the urethra and itching may occur. Some men also report a noticeable burning sensation in the urethra during urination, when urine irritates the inflamed urethral mucosa.
If we are talking about a venereal disease called "gonorrhea", caused by pathological microorganisms called gonococci, then the symptoms in the form of itching and burning with this pathology will be more pronounced, and the discharge will take on a purulent character: it will be sticky, thick, with a dirty yellow or greenish color, and a nasty putrid odor.
Transparent stretchy discharge in men can be observed with such a sexually transmitted pathology as chlamydia, the causative agents of which are considered to be chlamydia, but the addition of other types of infection cannot be ruled out. Most often, such a symptom as glassy discharge appears in the morning. Transparent discharge in the morning in men is called "morning drop", it can be either viscous glassy or watery. Sometimes it contains an admixture of pus or blood (in acute inflammation).
Discharge during chlamydia may also be accompanied by itching and burning in the urethra, back and lower back pain, an increase in temperature to subfebrile levels, and cloudiness of the first urine.
Ureaplasma and mycoplasma (causative agents are ureaplasma and mycoplasma, respectively) manifest themselves in men with the same transparent mucous discharge in small quantities. With ureaplasma, the discharge is transparent and is most often accompanied by pain and burning during urination. For a long time, the infection can be asymptomatic with good general health.
Mycoplasmosis is characterized by transparent and translucent discharge, as well as pain in the lower back, lower abdomen and penis area. During urination, a man may experience severe burning and pain. The disease may be accompanied by an increase in regional lymph nodes, swelling of the scrotum tissue, hyperemia of the genital area. Further, there is a decrease in sexual desire and problems with erection.
Transparent discharge is not typical for syphilis. Usually, this pathology, caused by pale treponema, is accompanied by abundant discharge of purulent fluid of whitish or yellowish color, which appears somewhere on the 20-21st day after infection, when other symptoms of the pathology are still absent.
In chronic STDs, the discharge is usually transparent without any impurities. But the acute stage is characterized by mucous discharge mixed with pus.
These pathologies are dangerous because their symptoms are not constant. Sometimes they disappear completely and the man forgets about them. But the disease remains and can lead to dangerous consequences.
Another very common sexually transmitted disease is trichomoniasis. In the male population, this disease very often does not cause any symptoms. But if they do appear, then first of all it is a burning sensation and transparent (and sometimes white, yellow or green) discharge from the urethra in men. Such discharge is transparent at the very beginning of the disease, later it changes color and acquires an unpleasant smell of rotten fish.
Transparent discharge with an unpleasant odor in men is also characteristic of a rare venereal pathology among men, called gardnerellosis. Again, there is a smell of rotten fish, the cause of which is the active reproduction of the pathogen - gardnerella, which, by the way, are also representatives of opportunistic microflora and reproduce only against the background of failures in the immune and other systems. Additional symptoms are itching and painful urination.
An unpleasant smell of discharge also appears with the development of inflammatory diseases in the area of the head and foreskin of the penis. With balanitis and balanoposthitis, an accumulation of the bacterial component is observed in the folds of the urethra. And the smell is the result of the vital activity of bacteria.
In any case, the appearance of an unpleasant odor in the discharge always indicates an infectious nature of the pathology, i.e. increased activity of bacteria and fungi. The latter are characterized by a sourish aroma.
There is also such a type of discharge from the urethra in men as prostatorrhea. In most cases, it is a whitish liquid, but sometimes transparent discharge is also observed. Prostatorrhea itself is not a pathology. And this symptom should not always be associated with any disease.
The appearance of prostate fluid (prostate secretion plus seminal vesicles without the absence of a bacterial component) can be observed during defecation against the background of chronic constipation, as well as after urination, especially after some abstinence. In this case, we are talking about defecation and micturition prostatorrhea, which are not considered signs of the disease.
It is not surprising if prostate secretion is released in small quantities after massage procedures in the area of the organ or after active physical exercise (this symptom often affects male athletes).
In other cases, the appearance of such discharge is associated with a decrease in the tone of the prostate gland or urethra, which occurs with diseases such as prostate hyperplasia (aka prostate adenoma), chronic prostatitis, impaired innervation of the bladder, etc. Moreover, prostatorrhea itself is observed in rare cases, usually accompanied by involuntary discharge of seminal fluid, and in some cases, pus.
Prostatorrhea may be accompanied by itching and discomfort in the urethra, and with some pathologies, more or less pronounced pain, burning, and stinging may also appear.
Transparent discharge during arousal in men in the vast majority of cases does not have a pathological cause. And such symptoms as pain, itching and burning do not occur, especially if the hygiene of the penis is at its best. But if such discharge is observed when a man does not experience sexual arousal, there is already something to think about.

Diagnostics transparent discharge in men
The danger of transparent discharge in men is that it can be both normal physiological manifestations and evidence of the development of one of the inflammatory or venereal pathologies. It is problematic even for a specialist to determine by eye what we are dealing with, not to mention the ignorant. True, in most cases, embarrassed by their problem, men wait until the last minute and rely more on the advice of close friends than on the opinion of a doctor.
Yes, it is not easy for men to talk about problems related to male dignity. And the very thought of diagnostics in the presence of female medical personnel is simply unbearable for a man. But when it comes to sexual and reproductive health, all embarrassment and prejudice must be discarded. It is better to make sure once that you are healthy, or to detect a disease in a timely manner, than to spend weeks and months to ensure that the pathology does not affect sexual function.
Men can contact a urologist or a specialist in male pathologies, an andrologist, with all their questions and problems, if such a doctor is available at the medical institution. After the man tells about his suspicions about the discharge from the urethra and describes the unusual symptoms, the doctor will begin a physical examination of the patient. In this case, he will pay most attention to the penis, prostate, and perineum.
It is possible to reassure those who consider the size of their penis to be insufficient, which is the cause of complexes. The length of the penis is the last thing that can interest a specialist doctor, even when he will be examining and palpating the organ. The doctor will be more interested in traces of discharge on the urethra, changes in its shape, the appearance of seals and signs of an inflammatory process, the presence of rashes on the skin, indicating allergies or venereal pathologies.
As for regional lymph nodes, the size will also be important for the doctor. In addition, important indicators are body temperature in their area, tenderness of the lymph nodes or absence of discomfort when pressing on them, mobility and presence of ulcers.
Once a man has visited one of his doctors (urologist or andrologist), the specialist will not fail to assess the situation with the health of the prostate, especially if the patient tells about the symptoms of possible inflammation. Palpation and massage of the prostate gland through the large intestine is carried out. Here the man will have to be patient and not be surprised when discharge from the penis appears, which the specialist needs for microscopic and morphological examination (for example, PRP test). It is important to know that before examining the prostate, doctors recommend refraining from visiting the toilet (meaning the act of urination).
The doctor must send the obtained biomaterial to the laboratory for testing. In addition, the patient is prescribed other tests: a general blood test, a blood test for glucose, a general and biochemical urine test. The tests must be taken in the morning after hygiene procedures, but before breakfast, i.e. on an empty stomach.
If there is a suspicion of oncological processes (detection of tumor formations, tissue dysplasia, enlarged lymph nodes, when they become hard but painless), material is taken for histological examination (biopsy).
Instrumental diagnostics of transparent discharge in men may include ultrasound examination of the bladder, kidneys, prostate gland, computed tomography of the pelvic organs, X-ray examination of the urinary tract (urography) if pathological processes in the kidneys and bladder are suspected.
Differential diagnosis
Differential diagnostics involves not only identifying the difference between physiological and pathological discharges, but also searching for the cause of these discharges. After all, the same symptom can indicate completely different pathologies. Therefore, it is very important to pay attention to other manifestations of diseases: changes in tissue density, homogeneity of these changes, the appearance of itching, burning, pain in the penis, as well as studying the results of laboratory tests.
For example, the prostate has not one, but two lobes. If they are enlarged equally with pronounced strands, we are talking about prostate adenoma, and if only one lobe or part of it is enlarged, oncology can be suspected, which is confirmed or refuted by the results of a biopsy.
As for the test results, an increase in the level of leukocytes indicates an exacerbation or acute stage of the inflammatory process. If a large percentage of eosinophils is detected in the blood, it is possible that the inflammation is of an allergic nature, but other causes of urethritis cannot be ruled out. If the inflammation is severe or is a consequence of a penile injury, erythrocytes can be found in the discharge. The same symptom is present in tumor pathologies, urolithiasis, and kidney inflammation.
Taking a smear for examination is a very important procedure that requires certain preparation. It is not recommended to urinate for 2-3 hours before taking the material for analysis. You should not get carried away with hygiene procedures either. You can do them in the evening, and go to the doctor's office in the morning without washing your genitals.
Treatment transparent discharge in men
It is important to understand that transparent discharge in men is not a pathology, but only one of the symptoms of a possible disease. This means that it is not the discharge itself that needs to be treated, but the disease in all its manifestations. We are not talking about physiological discharge, which does not require treatment, since it corresponds to the normal state of the male body, but about a pathological symptom that requires careful examination.
As we have already understood, in most cases of pathological discharge from the urethra, we are talking about an inflammatory process in it (urethritis). But there can be a great many reasons for the inflammatory process in the genital organ. If we are talking about non-bacterial urethritis (for example, a consequence of trauma), which is confirmed by smear tests, anti-inflammatory therapy is carried out and the patient is given instructions on how to lead a sexual life and care for the penis. In severe cases of traumatic urethritis, surgical treatment is prescribed.
If the inflammation was caused by venous congestion in the back of the organ (congestive urethritis, which often coexists with prostatitis caused by the same reason), the patient will simply have to change his lifestyle: return to traditional sex, treat constipation, replace physical inactivity with physical activity. Prostate massage may be prescribed in parallel.
If urethritis is of an allergic nature, the drugs of choice are antihistamines (Suprastin, Tavegil, etc.).
But in most cases, doctors still deal with urethritis caused by a bacterial infection. And here you can't do without antibacterial therapy. Another thing is that urethritis can be caused by many types of pathogens, and by and large, treatment should be prescribed after receiving the results of the analysis for the pathogen. But most often, no one waits 1.5-2 weeks for the results to come. Doctors prefer antimicrobial drugs with a wide spectrum of action.
True, in this case, attention should be paid to all the symptoms of the pathology in order to select an effective drug as accurately as possible. For example, if streptococci, staphylococci and other opportunistic microflora are suspected, the most effective prescription will be an antibiotic from the penicillin or cephalosporin series. In case of intolerance to these drugs or resistance of strains to beta-lactam antibiotics, macrolides may be prescribed, and in severe cases of purulent urethritis, fluoroquinolones.
Chlamydial infection (detected only by PCR diagnostics) can be treated with antibiotics of various groups:
- tetracyclines (“Doxycycline”),
- macrolides (“Erythromycin”, “Azithromycin”, “Josamycin”,
- fluoroquinolones (“Ofloxacin”, “Levofloxacin”).
In this case, preference is again given to drugs of the tetracycline series.
The same groups of drugs were previously used against mycoplasma, but recently the number of resistant strains has increased significantly, so we have to resort to the latest generations of fluoroquinolones (for example, Moxifloxacin). Of the tetracyclines, doxycycline-based drugs are quite effective. You can also turn to a new type of antibiotics - streptogramins (Piostacin, Pristinamycin), but they are not so easy to get in our country.
If the cause of inflammation in the urethra is ureaplasma, most likely we are talking about Ureplasma urealiticum, which is well treated with tetracyclines and macrolides (mostly, Josamycin is prescribed).
Metronidazole and Tinidazole have been used for many years to get rid of trichomonas. Their effectiveness does not decrease over the years.
Any other infections (even those whose pathogen could not be identified) are treated with tetracyclines. Other types of antibiotics are usually prescribed only when tetracyclines are ineffective.
If a person is prescribed a course of antibiotics (most often this is monotherapy or a combination of two effective antimicrobial drugs), they are also offered to take probiotics, which are able to normalize the beneficial microflora in the body that is disrupted by antibiotics.
It is pointless and even dangerous to treat viral urethritis with antibiotics. Antiviral drugs are intended to combat viruses (for example, "Acyclovir" if we are talking about herpes infection). But quite often the disease goes away on its own, it is enough to just increase immunity, which will not allow viruses to show excessive activity.
For both bacterial and viral infections, it is recommended to take immunostimulants, which help to increase immunity and give the body strength to fight the disease. For the same purpose, men are also prescribed vitamins (most often in the form of complexes containing various vitamin supplements and useful minerals).
Indicated for urethritis and local treatment with antimicrobial and anti-inflammatory agents. In this case, drugs can be administered directly into the urethra (instillations of Miramistin, Dioxidin, Furacilin, Hydrocortisone). As prescribed by a doctor, you can wash the penis with a solution of hydrocortisone, chamomile decoction, a weak solution of potassium permanganate, a solution of furacilin. These same agents can be used in the form of warm baths. For viral herpes infection, Acyclovir ointment is prescribed externally.
Physiotherapy treatment is always prescribed only for chronic pathologies. Acute infections and exacerbations of chronic inflammations are not treated with physiotherapy! Among the effective procedures, electrophoresis, UHF, laser and magnetic therapy can be distinguished. At the same time, all procedures must be carried out under the supervision of qualified medical personnel, after all, we are talking about male dignity, which is very sensitive to various external influences.
Drug therapy
It is clear that it is simply impossible to consider in one article all the drugs that are permissible for use in urethritis. And is it even necessary? It makes sense to focus on the drugs that doctors prescribe most often.
Doxycycline
An effective drug of the tetracycline series, used against various infectious agents, even those that are insensitive to other antibiotics. Active against opportunistic microflora, chlamydia, mycoplasma, syphilis and gonorrhea pathogens.
The drug is available in capsule form, which must be swallowed and washed down with water. It is recommended to do this after meals, so you can reduce the irritating effect of the drug on the gastrointestinal tract.
The initial dose of the drug is usually 200 mg. For mild infections, including chlamydia, mycoplasmosis and ureaplasmosis, the daily dose is then halved. It can be taken at once or divided into 2 parts and taken at intervals of 12 hours.
In therapeutic regimens, "Doxycycline" can be successfully combined with "Clindamycin" and "Gentamicin". The course of treatment in most cases does not exceed 2 weeks.
Syphilis is treated with a shock dose of 300 mg, which must be taken for at least 1.5 weeks.
Taking the drug may be accompanied by such unpleasant symptoms as nausea (sometimes vomiting), epigastric pain, diarrhea, increased sweating, dizziness. Allergic reactions can be both mild (in the form of itching and rashes) and severe (angioedema, anaphylaxis).
Contraindications to the use of the drug are: hypersensitivity to tetracyclines, porphyria, leukopenia, severe liver dysfunction. The drug is prescribed to children from the age of 9, when most of the teeth have already formed. During pregnancy, it can be used in the first trimester, but it is better to be careful after that. Breastfeeding is stopped during treatment with the drug.
Josamycin
Among the macrolides used in urology, the drug occupies one of the places of honor. It is effectively used for the eradication of various strains of chlamydia, mycoplasma and ureaplasma, which most often cause the appearance of transparent discharge in men.
The drug is available in the form of regular and dispersible tablets. The former should be swallowed and washed down with water, the latter can be used to prepare a suspension (dissolve in 20-40 ml of water). The medicine is taken outside of meals.
Men with urological problems are prescribed the drug in a daily dosage of 1 to 3 g, depending on the pathology and its course. It is recommended to divide the daily dose into 2 or 3 doses. The course of treatment is usually at least 5-7 days.
When treated with Josamycin, the patient may experience nausea, heartburn, and diarrhea. Appetite may decrease and gas formation may increase. Mild allergic reactions, liver dysfunction, pseudomembranous colitis, and hearing loss are also possible. If you do not take probiotics, dysbacteriosis and the activation of Candida-type fungi are quite likely.
The drug has few contraindications. It is not recommended to prescribe it for severe liver pathologies and individual intolerance to the components of the drug.
Metronidazole
An antiprotozoal and antimicrobial drug that is actively used for trichomonas infection and gardnerellosis, as it is active against the pathogens of these diseases.
It has several forms of release. For venereal pathologies, tablets, suspension, gel and cream for external use can be used, in severe pathologies - solutions for infusions and intravenous injections with subsequent transition to tablets.
Tablets for trichomonas infection and gardnerellosis should be taken in a dosage of 250 mg twice a day. In this case, the course of treatment will be exactly 10 days.
There is also an alternative regimen, according to which the drug is prescribed in a course of 5 to 8 days, but a single dose in this case will be 400 mg with the same frequency of administration.
Metronidazole tablets are swallowed without chewing. The medicine is taken during or after meals. The instructions suggest washing them down with milk.
Additionally, local therapy using a gel or cream may be prescribed.
Like any antibiotic, the drug has side effects. Patients may complain of diarrhea, loss of appetite, metallic taste in the mouth, development of inflammatory processes in the oral cavity and pancreas. Constipation and intestinal colic, dizziness and coordination disorders, headaches and sleep disorders are also possible. Inflammatory processes in the bladder, which may be accompanied by urinary incontinence, allergic reactions and joint pain, rarely occur. The level of leukocytes in the blood may decrease (leukopenia).
In what cases is the drug not prescribed? In case of individual intolerance to the components of the drug, organic pathologies of the central nervous system, epilepsy, in the first trimester of pregnancy and during breastfeeding. Carefulness with doses should be observed in case of serious pathologies of the liver and kidneys, especially if they occur with impaired functionality of the organ.
Hydrocortisone
A steroid drug with a pronounced anti-inflammatory effect, which is used for inflammation of the urethra and penis mainly in the form of external forms: ointment and solution.
