A decrease in the activity of alpha 2 -antiplasmin is observed in severe hepatitis, liver cirrhosis, chronic tonsillitis, DIC syndrome, thrombolytic therapy with streptokinase. In patients with chronic DIC syndrome, plasminogen is activated slowly and the content of alpha 2 -antiplasmin decreases sharply, which is associated with the rapid elimination of the alpha 2 -antiplasmin-plasmin complex. In patients with a low content of alpha 2 -AP-AP and reduced activity of the fibrin-stabilizing factor, the postoperative period may be complicated by bleeding.

An increase in the concentration of alpha 2 -antiplasmin in the blood is possible in patients with diabetes mellitus, who have had a streptococcal infection, with malignant neoplasms, acute thrombosis, and after surgical interventions.
