Peptika combipek is a medicine used for ulcers and GERD. It is a complex compound that helps destroy Helicobacter pylori.
Omeprazole creates a suitable environment for the development of antimicrobial activity, and at the same time acts directly against H.Pylori. [ 1 ]
Clarithromycin is a macrolide antibiotic; it inhibits protein binding within the bacterial cell by interacting with the 50S microbial ribosome subunit.[ 2 ]
Tinidazole shows activity against relatively simple bacteria, as well as most anaerobes.[ 3 ]
Omeprazole in combination with antibiotic treatment leads to rapid relief of ulcer symptoms and its healing.

Indications Peptica kombipek
It is used in the treatment of ulcers in the gastrointestinal tract or gastritis in a chronic form (associated with the influence of Helicobacter pylori) in people with some sensitivity to tinidazole.
Release form
The medicinal element is released in tablets and capsules.
Pharmacodynamics
Omeprazole is an antiulcer drug with antisecretory activity. It reduces the secretion of gastric hydrochloric acid and inhibits the effect of H+/K+- ATPase, preventing the development of the final stage of hydrochloric acid secretion.
Clarithromycin demonstrates an effect on Streptococcus agalactiae, Campylobacter jejuni, Staphylococcus aureus, Chlamydia pneumoniae with Haemophilus influenzae, Mycoplasma pneumoniae with Listeria monocytogenes, as well as gonococci and Moraxella catarrhalis. In addition, the list includes Ureaplasma urealyticum, Legionella pneumophila, Mycobacterium avium, Helicobacter pylori with Hansen's bacillus and whooping cough bacillus, as well as Propionibacterium acnes and Toxoplasma gondii.
Clostridia, gardnerella, peptostreptococci with bacteroides, fusobacteria and peptococci with eubacteria, intestinal lamblia, vaginal trichomonas and dysenteric amoebae are sensitive to tinidazole.
Pharmacokinetics
Omeprazole.
When administered orally, it is absorbed at high speed and completely. About 90-95% of the element is synthesized with proteins.
The half-life of the substance is 0.5-4.5 hours, but the antisecretory activity persists for 24+ hours.
The main part (77%) of omeprazole is excreted in urine. Among the excreted elements are hydroxyomeprazole with the corresponding carboxylic acid. The remainder of the portion is excreted in feces.
Clarithromycin.
After oral administration, clarithromycin participates in metabolic processes with the formation of the main metabolic element - 14-hydroxyclarithromycin. It has an antimicrobial effect similar to the active element in severity (or 1-2 times weaker; depending on the type of bacteria).
The half-life is approximately 3-4 hours. Approximately 20% of the substance is excreted unchanged via the kidneys, and another 15% is excreted as 14-hydroxyclarithromycin.
Tinidazole.
When taken orally, tinidazole is fully and rapidly absorbed. Protein synthesis is quite weak; the active element index is 88%.
Tinidazole penetrates into inflamed areas and various tissues without complications. Comparison of plasma values of substances in the cerebrospinal fluid allowed us to determine that tinidazole penetrates into it more actively than metronidazole (the figures are 88% and 43%, respectively). Tinidazole is detected in large quantities in mother's milk.
Over a 24-hour period, 20-25% of tinidazole is excreted in the urine.
Dosing and administration
1 combination pack of the medicine contains 2 capsules of omeprazole, and also 2 tablets of tinidazole with clarithromycin. One specified set is prescribed for 1-day intake.
It is necessary to take 1 capsule of omeprazole, as well as 1 tablet of tinidazole with clarithromycin in the morning, and then in the same amount in the evening. In addition, an additional intake of 0.25 g of clarithromycin in the morning and evening is mandatory.
The duration of the treatment cycle is 7 days.

Peptika combipek is not prescribed to persons under 14 years of age.
Use Peptica kombipek during pregnancy
It is prohibited to use the medication during pregnancy or breastfeeding.
Contraindications
Contraindicated in case of diagnosed intolerance to clarithromycin with tinidazole or omeprazole, and also in case of pathological blood changes. Also not prescribed in combination with ergot alkaloids.
Side effects Peptica kombipek
Side effects include nausea, metallic taste, headaches and vomiting.
Overdose
Poisoning when taken together with Mg and Al substances can cause thirst, symptoms of neurotoxicity, hypotension and flushing of the skin on the face.
Gastric lavage and supportive measures are required.
Interactions with other drugs
Administration of clarithromycin with theophylline may increase plasma levels of the latter.
The use of clarithromycin with terfenadine increases the plasma level of the latter, which may result in prolongation of the QT interval and the development of cardiac arrhythmia.
The combination of clarithromycin with orally administered anticoagulants (including warfarin) leads to an increase in the therapeutic effect of the latter.
The combination of clarithromycin with cyclosporine, cisapride, phenytoin and carbazepine, as well as with lovastatin, digoxin, disopyramide, astemizole and valproate with pimozide may provoke an increase in the plasma level of these substances.
Due to the intensive suppression of hydrochloric acid secretion processes, the element omeprazole is able to affect the absorption of ampicillin with ketoconazole and Fe salts.
Omeprazole reduces the rate of elimination of diazepam with phenytoin and warfarin.
The drug should not be used together with ergot alkaloid substances.
Storage conditions
Peptika kombipek should be stored in a dry place, out of the reach of children. Temperature level – within 15-25°С.
Shelf life
Peptika combipek can be used for a period of 36 months from the date of manufacture of the pharmaceutical element.

The analogs of the drug are Helicocin, Klatinol with Ezoxium combi, Pilobact and Ornistat with Beta-klatinol, as well as Pilokt.
