The arsenal of local anesthetics used in dentistry includes ointments for toothache on a gel base, which facilitates faster penetration of active substances.
Indications for the use of the ointment for toothache include relief of toothache, as well as pain during teething in children and third molars in adults, inflammatory diseases of the oral mucosa (stomatitis, gingivitis), gums and periodontium, in case of traumatic damage to the oral mucosa by dentures, etc.
Names of ointments for toothache: Kamistad gel (manufactured by Haupt Farma GmbH, Germany), Holisal (Jelfa SA, Poland), Kalgel (GlaxoSmithKline Pharmaceuticals, Poland).
Pharmacodynamics
The mechanism of the analgesic action of the Kamistad gel is provided by lidocaine hydrochloride, which, due to the blocking of sodium channels of nerve fibers, does not allow pain impulses to pass. The antiseptic effect of this ointment for toothache is provided by azulene of the extract of pharmacy chamomile, which is also included in its composition.
The pharmacodynamics of Holisal gel is based on the analgesic and anti-inflammatory action of the salicylic acid derivative - (2-hydroxyethyl) trimethylammonium salicyl (choline salicylate), which inactivates the cyclooxygenase enzyme COX1 and COX2 and reduces the production of lipid mediators prostaglandins. This ointment also contains a broad-spectrum antiseptic, cetyl chloride (ammonium derivative).
The active ingredients of Kalgel ointment are the anesthetic lidocaine and the cationic-surface antiseptic cetylpyridinium chloride.
Pharmacokinetics
These topical analgesics and antiseptics have little systemic absorption and manufacturers have not provided information on their pharmacokinetics.
Method of application and dosage of ointments for toothache
The method of application of Kamistad, Holisal and Kalgel gels is local: apply to the painful area with a clean finger and gently rub into the mucous membrane.
A single dose of Kamistad is a 5 mm strip of gel (use after meals, up to three times a day); the dose of Holisal is 1 cm for adults and 0.5 cm for children;
Kalgel has the same dosage, but it can be applied up to 6 times a day.
In the instructions for these drugs, an overdose is noted for the gel Holisal, which can enhance the effect of drugs taken to reduce fever and relieve pain.
An overdose of Kalgel is expressed in pallor, decreased pulse rate, difficulty breathing, and vomiting.
There have been no clinical cases of interaction with other drugs by the manufacturers of the listed medicines.
Storage conditions require a temperature not exceeding +25-30°C.
The shelf life of Kamistad gel is 5 years (opened tube - 12 months); Kalgel - 3 years, Holisal - 24 months.
Contraindications for use
Toothache ointments Kamistad and Holisal are not used in case of hypersensitivity to the components of the drugs; Holisal is not used for children under 12 years of age. Contraindications for the use of Kalgel: heart failure (2-3 degrees), abnormal heart rhythm (bradycardia), liver or kidney failure, low blood pressure.
Using toothache ointment during pregnancy: Kamistad and Kalgel are not used; Holisal - as prescribed by a doctor, with caution.
Side effects
Gel-based toothache ointments can have the following side effects: Kamistad and Holisal - local allergic reactions;
Kalgel - in addition to allergic reactions, difficulty swallowing.
