Ibuprom max is a non-steroidal drug with analgesic properties and belongs to a group of drugs that have a pronounced anti-inflammatory effect. In addition to acting as an analgesic and helping to relieve inflammation, its use helps to reduce hyperthermia accompanying fever. The drug also has an anti-exudative effect.

Indications Ibuprom max.
Indications for use of Ibuprom Max include the possibility of its use as an analgesic and anti-inflammatory agent for diseases of the musculoskeletal system that have a degenerative and inflammatory nature. These include, for example, arthritis of various types: chronic, psoriatic, rheumatoid, juvenile. In addition, the drug can be included in the complex therapy of arthritis in systemic lupus erythematosus. It is also justified to use it in fast-acting dosage forms when gouty arthritis occurs with an acute attack of gout.
It is advisable to prescribe Ibuprom max to patients with osteochondrosis, patients with Parsonage-Turner disease (neuralgic amyotrophy), and also with ankylosing spondylitis (Bechterew's disease).
The drug is highly effective as a pain reliever for pain of various etiologies, including: pain syndrome accompanying arthritis and arthralgia, myalgia, ossalgia, radiculitis. Ibuprom max helps reduce the intensity of migraine pain, toothache and headache, as well as those that accompany menstrual syndrome. This drug also acts as an effective analgesic used in oncological diseases. Indications also include the presence of neuralgia and neuralgic amyotrophy in the patient, also known as Parsonage-Turner disease; bursitis, tendinitis and tendovaginitis; pain syndrome in combination with inflammation in the postoperative period.
Thus, the indications for the use of Ibuprom max mainly consist of its inclusion in symptomatic therapy, as well as the need to reduce the intensity of pain and reduce the severity of inflammation at the time it is used. The drug does not have any significant effect on the course and prognosis of the disease.

Release form
The release form of Ibuprom max is presented in the form of tablets with a sugar coating, capsules with prolonged action, suspension intended for oral administration, tablets with prolonged action coated with a film, effervescent tablets, lozenges with mint and lemon flavor, respectively.
Ibuprom max is offered by the manufacturer as 1 sachet with 2 tablets placed in a cardboard package.
Further, the cardboard package may contain 1 blister with 10 tablets.
Sugar-coated tablets in 1, 2 or 4 blisters, respectively, each containing 6 tablets.
50 tablets make up the contents of a polyvinyl bottle packed in a cardboard box. The number of tablets in a bottle can also be equal to 24 pieces.
One film-coated Ibuprom Max tablet contains the main active ingredient - ibuprofen 200 mg, and also contains a number of auxiliary substances. Sugar-coated tablets are distinguished by the fact that each of them contains ibuprofen in a 400-milligram amount.
Thus, the most widely presented dosage form of this drug is film-coated tablets. They can be found in various packages and in different quantities depending on the particular package.
Pharmacodynamics
The main active ingredient of this drug - ibuprofen is a substance synthesized from propionic acid. The effect that the drug produces is due to the fact that its action disrupts the metabolism of arachidonic acid, as a result of the fact that the activity of the cyclooxygenase enzyme is inhibited. Cyclooxygenase itself acts as a catalyst for the process in which endogenous biologically active substances are synthesized in arachidonic acid. The effect of the drug on the human body consists in particular in inhibiting the processes in which thromboxane is synthesized, as well as prostaglandins E, F. As for the latter, a decrease in their number is observed both in the focus of the inflammatory process and in the tissues of the central nervous system. The use of the drug is characterized by the effect of non-selective inhibition of cyclooxygenase, which is due to its inhibitory effect on the activity of this enzyme in its two isoforms - cyclooxygenase-1 and cyclooxygenase-2, respectively.
When prostaglandins tend to decrease their presence in the inflammation focus, the receptors become less sensitive to the effects of chemical irritants. This in turn means that suppression of prostaglandin synthesis in the central nervous system provides a systemic analgesic effect. And by inhibiting prostaglandins in the hypothalamus, or more precisely in the center responsible for thermoregulation of the body, the drug is effective as an antipyretic in case of fever.
Finally, by acting as an inhibitor of the processes in which thromboxane is synthesized from arachidonic acid, Ibuprom max produces an antiplatelet effect.
Pharmacodynamics Ibuprom max is characterized by good absorption rates in the gastrointestinal tract. The degree of absorption of the drug reaches its maximum in the stomach and small intestine. The level of bioavailability is about 80 percent. However, it should be noted that despite the fact that absorption changes slightly with food intake, some slowdown in the absorption of the main pharmacologically active substance may occur.
In blood plasma, ibuprofen is 99%, i.e. almost completely subject to binding with proteins, among which albumin forms such bonds with it mainly. In order for the drug to reach its highest concentration in plasma after being taken orally, it takes from 45 minutes to an hour and a half. Synovial fluid begins to differ in the presence of the drug in its maximum concentration 2-3 hours after taking. There is a relationship between the maximum concentration in synovial fluid and changes in the maximum plasma concentration of the drug. The rationale for this is the fact that biological fluids differ in albumin concentration.
Metabolization processes occur in the liver, where the drug is hydroxylated and carboxylated with transformation into metabolites that do not have pharmacological activity.
The half-life of ibuprofen in its single 200-milligram dose occurs over 120 minutes. In case of higher dosage the half-life period increases. After repeated use of the drug it becomes equal to two to two and a half hours.
Pharmacodynamics of Ibuprom max in relation to its excretion and the body is that this process is carried out mainly by the kidneys. The drug is excreted by them as metabolites and only about 1% leaves the human body unchanged.
In some extremely small quantities, it is excreted in the bile in the form of metabolites.
Dosing and administration
The method of administration and dosage of the drug, as well as the length of time for which Ibuprom max is indicated, is determined by the attending physician, taking into account all the individual characteristics of each specific patient.
As a rule, this drug is prescribed to adults and children over 12 years of age in a dose of 200 to 400 mg three times a day.
Each dose must be separated from the previous dose by at least 4 hours. The interval between doses can be 4-6 hours.
Elderly patients do not require special dosage adjustments. As for patients with acute liver or kidney failure, the dosage should be reduced for them.
Ibuprom max is taken regardless of food intake. The tablet should not be divided into parts, it should be swallowed whole, washed down with water in the required amount.
The peculiarities of the use of the drug by patients with gastrointestinal diseases, as well as if there is a history of lesions of the duodenum and stomach of an erosive and ulcerative nature, is that Ibuprom max should be taken during meals. This rule applies to all dosage forms of the drug.
If headache appears as a side effect of ibuprofen, in order to relieve it, it is not allowed to increase the dose of this drug.
Use Ibuprom max. during pregnancy
It is recommended to exclude the use of Ibuprom max during pregnancy at the onset of the third trimester of gestation.
In relation to lactation and breastfeeding, it should be taken into account that due to the use of this drug, the presence of ibuprofen and its metabolites is noted in breast milk. Breastfeeding can be continued only if the drug is taken in a daily dose of no more than 1200 mg. However, if there is a need for a long-term therapeutic course using Ibuprom max or its use in large doses, breastfeeding should be discontinued.
Contraindications
Contraindications to the use of Ibuprom Max are based primarily on the patient's individual hypersensitivity to the effects of some of the components of the drug. This also includes poor patient tolerance to other non-steroidal drugs with anti-inflammatory properties.
Among the diseases in which the use of Ibuprom max is unacceptable, we note: the presence of allergic rhinitis, bronchospasm, urticaria. Both those that occur at the current time and those present in the anamnesis.
Classifies as unacceptable for the prescription of medicinal drugs the existence of a predisposition to the occurrence of gastrointestinal bleeding, the presence of non-specific ulcerative colitis, gastric ulcer and ulcerative lesions in the duodenum that aggravate the patient's medical history.

A carefully considered approach to the possibility of using the drug is also required for diseases affecting connective tissue, disorders of normal kidney and liver function, heart failure and arterial hypertension. Under regular medical supervision, this drug should be taken during the first two trimesters of pregnancy, as well as the period when a woman breastfeeds her baby.

Side effects Ibuprom max.
In some patients, the following side effects of Ibuprom max were observed in connection with the use of the drug.
The response, which was shown in particular by the gastrointestinal tract, was the occurrence of nausea, vomiting, heartburn, painful sensations in the area "under the spoon", digestive disorders, this medication is able to cause the phenomenon of flatulence, lead to stool disorders. In exceptional cases, gastrointestinal bleeding occurred, the formation of gastric ulcers and the development of ulcerative lesions of the duodenum were observed, colitis and ulcerative pancreatitis appeared.
Negative manifestations that occur in relation to the liver often include the occurrence of liver failure, increased enzymatic activity in the liver, and the development of hepatitis.
The central nervous system may respond to the use of Ibuprom Max in the form of headaches and dizziness, a state of excessive drowsiness.
In the cardiovascular system, negative changes such as increased heart rate and tachycardia are possible. Some cases were characterized by the fact that taking this drug caused heart failure and the development of hypertensive conditions. There is also a certain degree of risk of thrombotic arterial events, strokes, and myocardial infarction is not excluded.
The activity of the hematopoietic system may be affected, which may lead to the development of anemia, leukopenia, pancytopenia and thrombocytopenia.
The urinary system, exposed to the negative effects of the drug, caused swelling, increased urea levels in the blood with a general decrease in the amount of urine throughout the day.

Overdose
Overdose of Ibuprom max is characterized by the appearance of nausea, vomiting, gastralgia, dizziness and headache, lethargy and excessive drowsiness.
If the dose used is increased, the prerequisites for hypotension to begin and the potassium content in the blood to increase arise. Concomitant phenomena include fever, arrhythmia, metabolic acidosis, loss of consciousness up to a comatose state, impaired renal function and respiratory function.
The drug, when used irregularly in high doses over a long period of time, has been noted to cause hemolytic anemia, granulocytopenia, and thrombocytopenia.
Since there is no specific antidote for this medication, the main treatment measures are limited to gastric lavage and symptomatic therapeutic measures. Enterosorbents are also used.
In this case, hemodialysis is not justified due to the fact that the drug binds to an extremely high degree to proteins in the blood plasma.
It is necessary to monitor the body's vital functions until all symptoms of the negative effects caused by excessively high doses of the drug have been eliminated. Thus, electrocardiography and monitoring of changes in blood pressure are indicated. In addition, the necessary actions should be taken to prevent gastrointestinal bleeding and to exclude disorders in the central nervous system.
To avoid metabolic acidosis due to an overdose of Ibuprom max, it is necessary to promote the restoration and maintenance of the acid-base balance pH at a level of 7.0 to 7.5.
Interactions with other drugs
The interactions of Ibuprom max with other drugs are such that it is necessary to carefully weigh all the pros and cons of its use in combination with other non-steroidal anti-inflammatory drugs due to the increased likelihood of all kinds of negative consequences affecting the blood system and the gastrointestinal tract. Of the non-steroidal anti-inflammatory drugs, only those with a low level of systemic action are acceptable together with Ibuprom max.
The use of the drug is unjustified if glucocorticosteroids have already been prescribed, since this combination of these drugs causes a mutual increase in their toxicity.
When interacting with antihypertensive drugs, among which it is necessary to note, first of all, beta-blockers and ACE inhibitors, Ibuprom max reduces their effectiveness.
Anticoagulants used in combination with this drug require regular monitoring of blood clotting due to the fact that ibuprofen reduces platelet aggregation.
Medicines with diuretic properties, when combined with Ibuprom max, are characterized by a weakening of the effect they produce. This is most typical of thiazide and loop diuretics.
This medicine affects lithium preparations in such a way that their concentration in the blood increases. For this reason, it is necessary to conduct regular blood tests for lithium content.
Interactions of Ibuprom max with other drugs in some cases may both indicate the appropriateness of its use as part of a complex treatment, and determine the unjustification of its use with some of them. As, for example, in the case of zidovudine - an antiretroviral drug included in HIV therapy.
