"Hypoechoic formation" - this term is often used to describe the picture during an ultrasound examination. What can this mysterious term for an ordinary patient mean? First of all, do not panic: a hypoechoic formation in the mammary gland is not yet a disease or a diagnosis, but only a structural description of what the doctor sees when performing an ultrasound. Simply put, all tissues that have a relatively low density compared to other surrounding tissues have a hypoechoic structure.

A hypoechoic formation in the mammary gland is not always a symptom of a disease. However, in some cases, the presence of such a sign may indicate the presence of a fairly serious pathology.

Epidemiology

Hypoechoic lesions in the mammary glands are considered the most common pathologies among the female population. Thus, they are found in approximately half of all women examined.

The frequency of detection of hypoechoic formations reaches its maximum at 45 years of age and decreases upon reaching the premenopausal period.

Causes of a hypoechogenic mass in the breast.

In cases where an ultrasound examination reveals a hypoechoic formation in the mammary gland, the presence of the following diseases can be assumed:

  • glandular carcinoma is a hypoechoic formation with unclear and unclear contours, with the presence of an acoustic shadow, structurally heterogeneous;
  • adenosis is a hypoechoic formation with blurred contours, mostly of irregular shape;
  • A common cyst is a round, hypoechoic formation in the mammary gland with clear contours;
  • atypical cyst - a hypoechoic formation of the mammary gland with clear contours, a thick capsule and a large number of internal calcifications; usually at a late stage of the disease, the number of calcifications increases significantly;
  • fibroadenoma is a hypoechoic focal formation of the mammary gland with clear boundaries, but sometimes it can resemble a cancerous neoplasm with limited spread;
  • a benign tumor is an avascular hypoechoic formation of the mammary gland, the structure of which does not have a vascular network;
  • fibrocystic mastopathy is a hypoechoic homogeneous formation in the mammary gland, often multiple. In most cases, it has a hormonal etiology.

It should be noted that natural formations in the mammary gland, such as milk ducts or the vascular network, also have a hypoechoic structure.

Risk factors

There can be quite a lot of factors that contribute to the appearance of pathological hypoechoic formations in the mammary gland. They can be conditionally divided into several groups:

  1. Age factor: middle and old age.
  2. Gender factor: women get sick 100 times more often than men.
  3. Constitutional factor: overweight women get sick more often than thin women.
  4. Hereditary factor: if direct relatives had cases of mammary gland diseases, then they can recur in the following generations.
  5. Reproductive factor: early sexual development and lack of childbirth contribute to the development of diseases in the mammary gland.
  6. Metabolic factor: slowing down of metabolism, as well as its disruption, contribute to the development of breast diseases.
  7. External factors: hypothermia, chest trauma, unfavorable ecology, radiation can negatively affect the condition of the mammary glands.
  8. Internal factors: regular abuse of alcoholic beverages, frequent stressful situations can lead to the development of breast diseases.

Pathogenesis

The main mechanism of ultrasound diagnostics is the generation of high-frequency sound vibrations by an ultrasound sensor and their transmission to the tissues being examined.

The ultrasound sensor receives frequency reflection from the tissues and back - this phenomenon can be compared to the natural transmission of an echo. This is why ultrasound examination is often called echography.

The doctor performing the ultrasound examination can evaluate the image visible on the screen according to the following criteria:

  • the higher the frequency of the ultrasound waves, the clearer the image;
  • The lower the frequency of the waves, the deeper the penetration.

During the examination, the doctor evaluates the general location and size of the mammary glands, as well as their structure. The concept of "acoustic density" is usually used to evaluate the structure. This concept was created to describe the speed of ultrasound wave transmission in the tissue being examined. The degree of acoustic density (or, in other words, echogenicity) varies in different organs. A sonologist (ultrasound specialist) always knows the degree of echogenicity of all tissues in the body, so he or she immediately determines the presence of a norm or pathology.

Symptoms of a hypoechogenic mass in the breast.

What is meant by the term "hypoechoic formation in the mammary gland"? This description means that an area with reduced acoustic density has been detected in the mammary gland.

The first signs of such a zone are a darker (compared to nearby tissues) structure, through which ultrasound waves move more slowly than through nearby structures.

A liquid formation has similar characteristics. Most often, it is diagnosed later as a cyst - a cavity element with liquid contents inside.

Why doesn’t an ultrasound specialist immediately characterize a hypoechoic formation as a cyst?

The thing is that, unlike the term "hypoechoicity", a cyst is already a diagnosis, not a description. And to confirm the diagnosis, one ultrasound is not enough: additional tests and studies are required, which the doctor prescribes if a particular disease is suspected.

A round hypoechoic formation may eventually turn out to be:

  • cyst;
  • tumor;
  • echinococcal infection;
  • galactocele.

If the hypoechoic formation has unclear boundaries, then one can suspect:

  • fibroadenoma;
  • cancerous tumor.

Complications and consequences

The development of negative consequences and complications in the presence of a hypoechoic formation in the mammary gland depends on its structural features. Thus, the worst prognosis may be malignancy of the tumor - that is, its degeneration into a malignant neoplasm. This can happen, for example, with an untreated fibroadenoma: in one case out of 150, the disease degenerates into sarcoma.

Fibrocystic mastopathy can also in some cases be complicated by a cancerous tumor, but most often the disease takes a recurrent course and periodically "returns" despite the treatment. This happens when the cause of mastopathy is not established, or is established incorrectly.

Untimely treatment of any formations in the mammary glands can lead to the development of serious consequences. At the same time, if medical assistance is provided in time, a quick and complete cure of the disease is not excluded.

Diagnostics of a hypoechogenic mass in the breast.

If a hypoechoic formation is detected in the mammary gland, the doctor will definitely prescribe further diagnostic procedures that will clarify or refute the diagnosis.

  • Tests:
  1. a complete blood count, which will indicate the presence of inflammation and anemia;
  2. blood test for hormones;
  3. blood test for tumor marker CA-15-3.
  • Instrumental diagnostics:
  1. mammography;
  2. biopsy of tissue elements of the formation;
  3. computed tomography or magnetic resonance imaging;
  4. mammoscintigraphy.

Differential diagnosis

Differential diagnostics are usually performed between different types of hypoechoic formations in the mammary gland. The most common types of pathologies are cysts, malignant and benign tumors.

The treatment regimen for hypoechoic formations depends entirely on the final diagnosis established after a comprehensive diagnosis.

When benign cysts and other similar formations are detected, drug treatment is used, which may include the following medications:

  • Herbal medicines:
  1. Indinol - prescribed for fibrocystic mastopathy, 1 capsule daily with food. During the intake, the duration of the monthly cycle may change, as well as short-term abdominal pain.
  2. Fitolon – used for various breast tumors (including malignant ones), 1-2 tablets 2-3 times a day. The course of treatment is designed for 1-6 months. Sometimes the drug can cause an allergic reaction.
  • Hormonal agents:
  1. Utrozhestan is a gestagen that is prescribed for fibrocystic mastopathy - 200-300 mg per day. The drug can affect the course of the monthly cycle, and also cause headaches.
  2. Estrogel is an estrogenic agent that is used externally to treat estrogen-dependent tumors. One dose of the gel is applied once a day, daily, to the skin of the abdomen or inner thighs.
  3. Duphaston is a gestagen that is prescribed according to an individual scheme, according to the course of the menstrual cycle. The drug can cause headaches, migraines, sometimes nausea and menstrual irregularities.
  • Anti-inflammatory drugs:
  1. Ibuprofen - prescribed for inflammatory infiltrates and pain syndrome, 200 mg up to 4 times a day. The use of the drug is sometimes accompanied by abdominal pain, dyspepsia. Pancreatitis may develop.
  2. Wobenzym is an anti-inflammatory drug that is often prescribed for mastopathy. The average dosage of the drug is 3 tablets three times a day.

Treatment of hypoechoic lesions in the mammary gland is prescribed only after a final diagnosis has been made. As we mentioned at the beginning of the article, the term "hypoechoic lesions" is only a sign of a possible disease, or a structural description of the image obtained on an ultrasound machine.

Vitamins

As for taking vitamins, it is advisable to consume them mainly in the form of plant products: fruits and vegetables.

For example, vitamin A – retinol – is present in large quantities in carrots, peaches, tomatoes, apricots, gooseberries. Retinol has a direct effect on cell growth, facilitates liver function and hormone production.

Almost all plant products contain vitamin C to a greater or lesser extent – ascorbic acid, which potentiates oxidation-reduction reactions, improves the body’s energy supply, and promotes the elimination of toxic substances.

Vitamin E – tocopherol – can be obtained from vegetable oils, milk and eggs. Tocopherol provides protection for healthy cells from the effects of free radicals and also normalizes protein production.

If pathological hypoechoic formations are detected in the mammary glands, it is recommended to drink at least 100 ml of fresh vegetable or fruit juice daily. At night, you can additionally drink a cup of dried fruit compote or a fermented milk product.

Physiotherapy treatment

If hypoechoic formations are detected in the mammary gland, it is strictly forbidden to use physiotherapy in the chest area until a final diagnosis is established that does not prohibit such procedures.

The fact is that a hypoechoic formation may ultimately turn out to be a malignant tumor or have a high risk of malignant degeneration. And physiotherapy in this case can act as a catalyst for malignant processes, so such procedures are never prescribed before a final diagnosis is made.

Folk remedies

Is it possible to use traditional medicine recipes if a hypoechoic formation is found in the mammary gland? Let's be honest: until a final diagnosis is made, any treatment measures should be postponed, including those based on traditional methods of treatment.

If a hypoechoic formation is detected in the mammary gland:

  • You cannot apply hot compresses, go to a sauna or sunbathe;
  • Do not apply warming ointments;
  • You cannot use alcohol-based lotions and compresses.

You should always wait until the doctor makes an accurate diagnosis, and only then, after consulting with the same doctor, begin treatment.

  • At night, you can apply a burdock or coltsfoot leaf to the sore breast, and in the morning, wipe the breast with a damp cloth and apply a little burdock oil to it.
  • You can prepare a mixture of equal parts of ground fresh burdock root, honey and castor oil. Spread the mixture evenly on a linen napkin and apply to the mammary gland overnight.
  • It is useful to apply compresses of grated fresh pumpkin pulp at night.
  • You can take a fresh cabbage leaf, apply a little fresh butter to it, sprinkle with fine salt and apply to the affected breast. It is also better to use such a compress at night.

Herbal treatment

  • If you pour boiling water over the wormwood leaves and wrap them in a linen napkin, then such a poultice can be applied to the sore chest in the morning and evening, for about 1 hour. The same leaves can be used up to 4 times.
  • You can make a compress based on St. John's wort infusion: pour 1 tbsp of the herb with 1 glass of boiling water and leave for 40 minutes. Then soak a napkin in the infusion and apply to the affected area. Sometimes other plants are used instead of St. John's wort - for example, knotweed, sweet clover or gentian.
  • Compresses prepared on the basis of clover infusion have a good antitumor effect. To prepare such an infusion, take 2 tbsp. clover flowers and pour 250 ml of boiling water over them. After 10-15 minutes, the preparation can be used for compresses.
  • The burdock leaf is rolled through a meat grinder. The resulting mass is applied to the affected area, covered with a towel on top. This is recommended for treating, for example, mastopathy.

Homeopathy

Homeopathic treatment can be used as an adjunct – that is, as an addition to the main treatment. For diseases of the mammary glands, which are accompanied by the appearance of hypoechoic formations, it is recommended to use the following homeopathic remedies:

  • Mastodinone – take for at least 3 months, 1 tablet twice a day.
  • Cyclodinone – take 1 tablet daily for 3 months.
  • Mastopol - standard dosage of the drug - 1 tablet three times a day half an hour before meals. Duration of treatment - 2 months.
  • Mastiol-Edas 127 - unless the doctor has prescribed another treatment regimen, take 5 drops of the drug in 1 teaspoon of water three times a day. Duration of treatment is 3 months (with breaks for menstruation).

The listed homeopathic preparations are practically free of negative side effects. Only rarely are cases of allergic reaction to the drug possible.

Surgical treatment

Surgery is most often prescribed when a hypoechoic formation in the mammary gland turns out to be a malignant tumor: in this case, surgical intervention can be considered as an independent method of treatment, or in combination with radiation and chemotherapy. A comprehensive approach usually allows achieving the highest rate of healing.

Surgical treatment as an independent method is carried out in patients with benign tumors that do not respond, or respond poorly, to drug treatment.

However, sometimes the operation may be contraindicated: contraindications may include advanced age of the patient, heart and vascular diseases in the decompensation stage, as well as advanced stage of malignant tumors with mass metastasis. The best effect after the operation is expected if the treatment is carried out in the early stages of tumor development, taking into account the comprehensive approach to the problem.

Prevention

Leading a healthy lifestyle is the best way to prevent the development of various diseases, including breast diseases.

It is recommended to move more, eat properly and in a balanced manner, get adequate rest, and avoid stressful situations and injuries.

For healthy breasts, it is important not to wear tight and uncomfortable bras – underwear should be loose, but at the same time provide good support for the breasts.

Bad habits, such as smoking and drinking alcohol, increase the risk of breast disease. Negative factors also include:

  • proximity to highways;
  • radiation;
  • harmful industries.

You should not take contraceptives for a long time without a doctor's prescription, as they can significantly change hormonal levels, which is the cause of various breast tumors.

You should undergo regular medical examinations with a gynecologist: early detection of various hypoechoic formations in the mammary gland increases the likelihood of a favorable outcome.

Forecast

The presence of a hypoechoic formation does not mean the development of a malignant process. However, such a sign may indicate the presence of other, no less complex diseases. Only the use of complex diagnostics will help to accurately determine the pathology.
