Enadipine is a complex drug that has hypotensive properties.

Indications Enadipine

It is used to reduce elevated blood pressure, as well as for the treatment of angina pectoris and variant angina pectoris.

Release form

The release of the therapeutic agent is done in tablet form.

Pharmacodynamics

Enadipine is a combined agent with an antihypertensive effect, containing enalapril with amlodipine. The medicinal effect is provided by the properties of its active elements.

Amlodipine inhibits the activity of slow Ca channels, and also blocks the penetration of calcium ions through the walls of slow Ca channels into vascular cells and smooth muscle cardiomyocytes. As a result, the overall resistance of peripheral vessels decreases, which leads to a decrease in blood pressure.

This does not lead to a change in the heart rate, and as a result of a decrease in the load with respect to the cardiac musculature, the need for a heart in oxygen decreases.

The therapeutic effect allows expanding the main arterioles with coronary arteries - inside the affected ischemia and healthy zones of the myocardium. This increases the passage of oxygen inside the myocardium, and in people with variant angina prevents the appearance of coronary spasms. In individuals with increased blood pressure, a one-time daily portion of drugs leads to a decrease in these indicators by 24 hours. Amlodipine has a gradual onset of exposure, which is why it does not cause the appearance of signs of acute hypotension.

In people with angina, taking a single dose of Enadipine per day increases tolerance for physical exertion, reduces the frequency of angina attacks and the number of nitroglycerin tablets taken.

Enalapril maleate is the agent of ACE inhibitors. It inhibits the formation of angiotensin 1, as well as 2. During its action, the overall resistance of peripheral vessels, the level of blood pressure, as well as pre- and post-loading with respect to the myocardium decreases. In addition, the substance reduces the rate of degradation of bradykinin and enhances the binding of PG.

Prolonged use leads to a weakening of myocardial hypertrophy, and besides this myocytes are arterial membranes of a resistive nature. The component contributes to the improvement of blood supply to the ischemic zones of the myocardium. Together with this, it inhibits the progression of left ventricular dysfunction in people who have had previous myocardial infarction.

The peak of antihypertensive effect is observed after 4-6 hours and lasts for 12-24 hours.

Pharmacokinetics

Pharmacokinetic characteristics of drugs are due to the interaction of its elements. Peak values of amlodipine inside the blood plasma are noted after 3-4 hours after drug use and remain at this level within 24 hours. Eating does not affect the degree of absorption of the substance inside the stomach. Stable enalapril indices inside the plasma are recorded after 7-8 days.

Metabolism of Enadipine develops inside the liver, and the excretion of the drug mainly flows through the kidneys.

In elderly people there is a decrease in the level of clearance, because of which the plasma half-life of drugs can increase.

Dosing and administration

You can consume the drug without tie to the reception of food, at the same time washing down with tablets 0.5 glasses of plain water.

The size of the drug dosage should be selected separately for each patient, given its condition and the nature of the disease. Adults are usually prescribed a 1-hour drug tablet once per day. Taking into account the patient's individual sensitivity, a maximum of 2 tablets of drugs are allowed per day.

Use Enadipine during pregnancy

There is no data on the safety of use of drugs in pregnant women or breast-feeding mothers.

Contraindications

The main contraindications:

  • presence of intolerance in relation to the elements of the medication;
  • Stenosis affecting the aortic aperture;
  • hypertrophic form of cardiomyopathy, accompanied by obstruction of the left ventricular exit tract;
  • decreased blood pressure;
  • shock cardiogenic character;
  • 2-sided or 1-sided stenosis affecting the arteries inside the kidneys;
  • gingivitis;
  • patients who have recently been transplanted to the kidney.

Side effects Enadipine

The intake of tablets can cause the appearance of such side effects: a feeling of heat, hot flashes, headaches, orthostatic hypotension, a decrease in blood pressure, dizziness and tachycardia. In addition, swelling of the legs, allergy symptoms (itching, rashes, skin hyperemia, urticaria and Quinnke edema), vomiting, dry type cough, myositis, arthritis, and also nausea, arthralgia, Stevens-Johnson syndrome and hepatic impairment may occur. Continuous use leads to paresthesia, a feeling of severe fatigue or weakness, as well as pain in the limbs.

Occasionally an increase in the indices of creatinine with urea within the blood plasma is observed. After the end of the use of drugs, these indicators return to normal values. Such negative symptoms are usually noted in people with renal problems or when combined with diuretics, people with heart failure.

Overdose

Most often, intoxication symptoms are marked: a strong decrease in blood pressure (approximately 6 hours after drug use), a state of stupor and tachycardia.

For the treatment of disorders, gastric lavage, maintenance of the CCC, control of pulmonary and cardiac function are prescribed, and in addition monitoring of the values of bcc and diuresis. It is also necessary to keep the limbs of the patient in a raised state and administer intravenously dopamine with calcium gluconate.

Interactions with other drugs

Combination of the drug with potassium diuretic drugs (such as spironolactone and triamterene with amiloride) is prohibited, because this can increase plasma potassium levels. With such treatment it is required to monitor the plasma values of potassium.

Use of the drug along with drugs blocking the channels of Ca, diuretics, hydralazine, and in addition to β-adrenoblockers and prazosin potentiates the antihypertensive effect.

A combination of Enadipine with NSAIDs may lower the therapeutic activity of the medication.

The combination with lithium drugs can cause a delay in the excretion of lithium, which increases the likelihood of developing toxic effects.

Simultaneous administration of the drug with warfarin leads to a slight change in the effect of warfarin on the performance of the new PTV.
