Dapril belongs to a group of drugs from a group of synthetic and natural chemical compounds that are usually used as a preventive and therapeutic agent for high blood pressure or heart and kidney failure.

Dapril is effective in cases of high blood pressure, moderate renal or cardiac insufficiency. At the beginning of treatment, the drug may cause a persistent decrease in blood pressure, which may affect the patient's condition (special attention should be paid to people whose work involves operating machinery or driving a car).

Pharmacodynamics

Dapril blocks the formation of oligopeptide hormone, which has a vasoconstrictor effect. Also, a decrease in total peripheral vascular resistance, pre- and post-load on the heart is noted, there is practically no effect on the heart rate and minute blood volume.

In addition, the resistance of the renal vessels decreases and blood circulation in the organ improves. In most cases, a decrease in pressure after taking the medication is noted after 1-2 hours (maximum after 6-9 hours).

The supporting therapeutic effect is observed after 3-4 weeks from the beginning of treatment. The drug withdrawal syndrome does not develop.

During treatment, an increase in the undemanding nature of physical activity is noted, while in patients with high blood pressure, a decrease in pressure is observed without the development of reflex tachycardia.

Pharmacokinetics

Dapril is absorbed by approximately 25-50%. The degree of absorption of the drug is not affected by food intake.

In blood plasma, the drug reaches its maximum concentration after 6-8 hours.

There is no binding of the drug substances to proteins and no metabolism; the drug is excreted unchanged by the kidneys.

In case of renal dysfunction, the period of drug elimination increases in accordance with the degree of functional impairment.

Dosing and administration

Dapril for high blood pressure is usually prescribed at the beginning of the course at 5 mg per day in one dose. If the therapeutic effect is not observed, the dosage is increased to 10 mg once a day. The interval between dose increases should be at least three weeks. It is recommended to take no more than 40 mg per day.

In chronic heart failure, diuretics should be discontinued several days before taking Dapril. The initial dosage is 2.5 mg per day. Depending on the individual response of the body, the dosage can be gradually increased. No more than 20 mg should be taken per day.

In old age and renal failure (moderate) at the beginning of treatment 2.5 mg per day is prescribed. As a rule, in this case the specialist prescribes the dosage individually (no more than 20 mg per day).

Contraindications

Dapril is contraindicated in cases of hypersensitivity to some components of the drug, history of Quincke's edema, narrowing of the renal arteries, aortic walls, mitral valve, pregnancy, Conn's syndrome, and in children under 12 years of age.

Side effects from the respiratory system (dry cough, runny nose, dry mouth) may also be observed.

After taking the medication, you may experience nausea, headaches, vomiting, stomach pain, loss of appetite, constipation, increased fatigue, dizziness, tinnitus, insomnia (in rare cases, cholestatic jaundice may occur).

Interactions with other drugs

When Dapril is taken simultaneously with other drugs that lower blood pressure (especially diuretics), an increased hypotensive effect is observed.

Non-steroidal drugs with anti-inflammatory effect (acetylsalicylic acid, ibuprofen, etc.), sodium chloride with Dapril reduce the therapeutic effect of the latter.

Taking the drug simultaneously with potassium or lithium leads to increased levels of these substances in the blood.

Immunosuppressive drugs, antitumor agents, allopurinol, steroid hormones, procainamide in combination with Dapril lead to a decrease in the level of leukocytes.

Dapril increases the manifestation of alcohol poisoning.

Narcotic drugs and painkillers enhance the therapeutic effect of Dapril.

Anaphylactic reactions are possible with artificial blood purification.
