The cut-off point is considered to be the content of antibodies to tissue transglutaminase in the blood serum above 10 IU/ml for the IgA class, and above 10 IU/ml for IgG.
Tissue transglutaminase belongs to the family of calcium-dependent acyltransferases that catalyze the formation of cross-links between proteins. It has now been established that tissue transglutaminase is the main, if not the only, endomysial antigen in patients with celiac disease.
Determination of antibodies to tissue transglutaminase in blood serum is a highly specific method for diagnosing gluten enteropathy. Indirect immunofluorescence and ELISA are used to detect antibodies. Antibodies to tissue transglutaminase can be detected in more than 95% of patients with celiac disease, and their concentration in blood serum correlates with the presence or absence of gluten in food. IgA antibodies have 95-100% sensitivity and 90-97% specificity.
