It is used to restore blood processes inside the vessels by lysis of the thrombus during myocardial infarction, which is acute.
It is also used in cases of pulmonary embolism accompanied by hemodynamic disorders.
Can be prescribed in the acute phase of ischemic stroke.
Release form
The drug is released in the form of an infusion lyophilisate, inside sterile glass vials with a capacity of 50 ml. The box also contains a solvent - in vials with a capacity of 50 ml.
Pharmacodynamics
The drug has a thrombolytic effect and acts as a tissue activator of the element plasminogen (a glycoprotein that helps modify the drug into plasmin). As a result, an anticoagulant and fibrinolytic effect develops.
The fibrinolytic activity of the drug is limited to the thrombus area, and it has almost no effect on other hemostasis processes (fibrinogen is not destroyed, and systemic fibrinolysis is not activated). Due to this, the likelihood of bleeding does not increase, and hypofibrinogenemia does not develop.
If an acute stage of pulmonary embolism develops, against the background of which hemodynamic disorders are observed, the use of Actilyse leads to a short-term decrease in the size of the thrombus, and in addition to stabilization of the pressure inside the pulmonary artery and the maintenance of physiologically adequate right ventricular function.
Pharmacokinetics
The drug is excreted from the bloodstream fairly quickly. The half-life is approximately 5 minutes, so after 20 minutes only about 10% of the initial levels remain in the blood. The remains of alteplase, distributed within fluids with tissues, are excreted within 40 minutes. The process of biotransformation of the drug occurs in the liver.
Dosing and administration
Preparation of infusion fluid: the lyophilisate from the vial is diluted in sterile liquid, which is contained in the 2nd vial from the pack, until a concentration of 1 mg/ml is obtained. After that, this liquid can be diluted using a physiological solution. The medication is administered intravenously. The portion size is selected taking into account the patient's condition and weight, as well as the nature of the disease.
During the acute phase of infarction (the first 6 hours), people weighing over 65 kg should be given 15 mg of the drug intravenously by jet stream; then 50 mg is administered via infusion (over 0.5 hours), and another 35 mg (over 60 minutes) until the dose reaches 0.1 g. After 6-12 hours from the onset of symptoms, 10 mg of the substance is administered intravenously by jet stream; then, 50 mg of the drug is infused over 1 hour, and later 10 mg (at 30-minute intervals) until the total dose reaches 0.1 g over a 3-hour period.
Persons weighing less than 65 kg should be administered a maximum of 1.5 mg/kg of the drug. Aspirin and heparin are also administered together with Actilyse. During the use of the drug, it is necessary to monitor the APTT values.
During therapy for pulmonary embolism, the medication is administered intravenously by jet stream - in a 10 mg dose, over 1-2 minutes, then in a dosage of 90 mg over 120 minutes, until a total dose of 0.1 g is obtained. People weighing less than 65 kg are allowed to use a maximum of 1.5 mg/kg of the medication.

Use Actilize during pregnancy
During pregnancy or lactation, the medication should be used with great caution.
Contraindications
Among the contraindications:
- the presence of severe sensitivity to the drug;
- neoplasms affecting the central nervous system;
- hemorrhagic form of diathesis;
- surgical procedures in the spinal cord area;
- pericarditis;
- diseases in which there is a disorder of cerebral circulation (hemorrhagic stroke, as well as intracranial hemorrhage);
- vascular aneurysm;
- pancreatitis;
- retinopathy of a hemorrhagic nature;
- obstetric labor or general surgical operations;
- a pronounced increase in blood pressure values;
- endocarditis of bacterial origin;
- aggravated ulcer;
- liver cirrhosis;
- varicose veins affecting the venous vessels in the esophagus;
- liver failure;
- portal hypertension;
- hepatitis.
Caution is required when prescribing drugs to elderly people, as well as to people with conditions that pose a high risk of bleeding (biopsies or punctures in the vascular area, as well as intramuscular injections).
Side effects Actilize
The use of the drug sometimes leads to the following side effects:
- local external bleeding in the area of damaged vessels, and also from the nose, gums or the puncture area;
- bleeding of an internal nature (urogenital system, gastrointestinal tract, as well as parenchymal organs);
- arrhythmia;
- Vomiting, decreased blood pressure, nausea and thromboembolism are occasionally observed.

Overdose
In case of poisoning, complications of a hemorrhagic nature are observed - internal or external bleeding.
Interactions with other drugs
Using the medication together with other drugs that affect platelet function or blood clotting increases the likelihood of bleeding.
The combination of Actilyse with ACE inhibitors increases the likelihood of anaphylactic symptoms.
The infusion fluid of the drug must not be mixed in the same container with other medicinal products.
Storage conditions
Actilyse should be kept at a temperature of 25°C. The prepared infusion fluid can be kept in the refrigerator for a maximum of 24 hours.
Shelf life
Actilyse can be used within 36 months from the date of manufacture of the therapeutic agent.
Application for children
Actilyse is used with caution in pediatrics.

An analogue of the drug is Alteplase.
Reviews
Actilyse receives mostly positive reviews from medical professionals. The drug has many advantages, the main one being its fibrin specificity. The lack of production of antibodies against the drug allows for its repeated use; it is also noted that it only occasionally leads to a decrease in blood pressure values.
The use of Actilyse in case of a heart attack (with rapid initiation of therapy – during the first 6 hours) significantly reduces the mortality rate from this disease. At the same time, the administration of the drug often helps to avoid the development of cardiogenic shock, which is a severe complication of a myocardial infarction.
