Tenoric is a complex antihypertensive drug with a fairly long-lasting therapeutic effect.

It is used to reduce excessively high blood pressure.
Release form
The drug is released in tablet form, inside blister packs or strips, with 28 or 100 tablets inside a pack.
Pharmacodynamics
Tenoric contains the element atenolol, which is a cardioselective β-blocker and has a hypotensive effect, and in addition chlorthalidone, which has diuretic properties. Both of these components have a long medicinal half-life, which allows for an antihypertensive effect lasting at least 24 hours.
Atenolol is a synthetic cardioselective adrenergic receptor blocker that does not possess membrane stabilization or partial sympathomimetic agonist effects.

Absorption and distribution processes.
After oral use, the active element is absorbed in the gastrointestinal tract by approximately 45-50%. The Cmax level of this component in the blood plasma is recorded after 2-3 hours after use. Synthesis with protein is quite weak - only about 5-15%.
Metabolic processes and excretion.
It is known that atenolol is not metabolized in the liver. More than 90% of the substance absorbed into the bloodstream is excreted completely unchanged.
The half-life is approximately 6-9 hours, but in cases of severe renal failure, this figure may increase, because atenolol is excreted mainly through the kidneys.
Chlorthalidone
Absorption and distribution processes.
After oral administration of chlorthalidone, about 60-65% of the substance is absorbed in the gastrointestinal tract. Plasma Cmax values are noted approximately after 10-12 hours. Chlorthalidone has a fairly strong synthesis with protein - about 70-75%.
Excretion.
It is excreted through the kidneys, and the half-life of the substance ranges within 50 hours.
Dosing and administration
The average dosage for an adult is 0.1 g per day. However, it is still recommended to start treatment with a single dose of 0.05 g of the drug per day.
For elderly people, it is recommended to use lower dosages of the medication.
People with kidney problems should reduce the frequency of drug use if such a need arises.
It is also necessary to take into account that with long-term use of Tenoric, its withdrawal should be done gradually, without stopping the intake too abruptly.
Use Tenorica during pregnancy
The use of Tenoric during pregnancy is permitted only if there are vital indications.
Contraindications
Main contraindications:
- bradycardia of high intensity;
- cardiogenic shock;
- decreased blood pressure values in a pronounced or progressive form;
- pheochromocytoma;
- metabolic form of acidosis;
- severe disorders of peripheral blood flow function;
- AV block of 1st or 3rd degree;
- SSSU;
- diabetes mellitus or hypoglycemia;
- heart failure (chronic or acute stage);
- variable angina;
- bronchial asthma, which has a progressive form;
- obstructive bronchitis;
- myasthenia gravis or gout;
- hepatitis in its acute form;
- acute renal failure;
- the presence of hypersensitivity to medicinal elements.
Side effects Tenorica
Taking the medication may cause the occurrence of certain side effects:
- disorders in the cardiovascular system: development of bradycardia, orthostatic collapse, Raynaud's disease, arrhythmia and AV blocks, as well as increased manifestations of cardiac failure, cold extremities and the appearance of signs of intermittent claudication;
- disorders affecting the functioning of the PNS or CNS: headaches, confusion, mood lability, dizziness, hallucinations, as well as acute psychosis, sleep disorders, paresthesia, apathy, visual disturbances, increased fatigue and a feeling of disorientation;
- problems with digestive function: gastrointestinal disorders, dry mouth, nausea (due to the effects of chlorthalidone), hepatotoxicity due to intrahepatic cholestasis, pancreatitis, increased liver transaminase levels, anorexia and constipation;
- disorders of hematopoiesis: agranulocytosis, thrombocyto- or leukopenia, eosinophilia or purpura;
- lesions affecting the epidermis: dry eye membranes, exacerbation of psoriasis or psoriasis-like symptoms, as well as alopecia, photosensitivity and rashes;
- respiratory dysfunction: bronchial spasms;
- laboratory test results: hypokalemia, hyperuricemia or hyponatremia;
- others: decreased potency, increased levels of antinuclear antibodies and impaired glucose tolerance.
But in general, Tenoric is often tolerated by patients without significant complications. Negative manifestations occur rarely and have a very weak degree of expression, being, for the most part, transient.

Overdose
Intoxication with the drug can cause intense bradycardia, acute cardiac failure, decreased blood pressure, convulsions with bronchial spasms, and a strong feeling of drowsiness.
To eliminate such symptoms, sometimes even hospitalization with placement in the intensive care unit is required, where gastric lavage is performed under the supervision of doctors.
In case of a significant decrease in blood pressure and severe shock, it is necessary to administer plasma or a plasma substitute to the victim.
When bronchial spasms develop, bronchodilators are used.
If necessary, hemoperfusion or hemodialysis procedure can be performed.
Interactions with other drugs
When combining the drug with dihydropyridine (the substance nifedipine), as well as its derivatives, the likelihood of a decrease in blood pressure may increase, and in people with latent heart failure, such a combination can lead to severe circulatory disorders.
In addition, it is necessary to avoid combining the substance with CG and β-blockers, because this can significantly increase the AV conductivity index.
In addition, β-blockers can cause an exacerbation of rebound hypertension, which often occurs when clonidine is abruptly discontinued. Provided that the treatment regimen involves the use of both drugs, β-blockers should be discontinued several days before clonidine is discontinued. If clonidine needs to be replaced with a β-blocker, the latter should be started a couple of days after clonidine has been discontinued.
It should be noted that β-blockers should be combined with category 1 antiarrhythmic drugs with extreme caution, because such combinations can lead to a summation of the cardiodepressant effect.
The therapeutic effect of β-blockers can be neutralized and blood pressure values can be significantly increased by using them together with certain sympathomimetics – norepinephrine (noradrenaline) or epinephrine (adrenaline).
Such drugs as indomethacin and ibuprofen (substances from the group of NSAIDs and salicylates) lead to a weakening of the antihypertensive properties of β-adrenoblockers. In addition, when using salicylates in high doses, the toxic effect of these elements on the central nervous system increases.
The use of medications containing lithium should also be excluded if diuretic drugs are used simultaneously with them. Due to such a combination, the values of lithium clearance in the kidneys decrease.
An increased probability of a decrease in blood pressure (or a sharp drop in these indicators) can be caused by a combination of β-blockers with general anesthetic drugs. In addition, there is a risk of potentiating the properties of curare-like muscle relaxants.
The use of Tenoric in combination with MAOIs may lead to an increase in blood pressure.
After the simultaneous use of the drug with ACE inhibitors (such as enalapril or captopril) at the initial stage of treatment, a sharp increase in the hypotensive effect can be expected.
Combined administration with furosemide, GCS, and amphotericin B leads to increased potassium excretion.
The medicinal effectiveness of insulin and the effect of oral antidiabetic drugs may be weakened when combined with Tenoric. In this regard, people taking these medications need to constantly monitor blood sugar levels.
Certain antihypertensive substances (including barbiturates, tricyclics, diuretics and phenothiazines with vasodilators) can potentiate the antihypertensive properties of Tenoric.
The use of β-blockers simultaneously with drugs that block the activity of Ca channels leads to the development of a negative inotropic effect and enhances its effect. Extreme caution is necessary for people with reduced contractile function of the myocardium and disorders of AV and sinoatrial conduction, because such combinations can lead to the development of intense bradycardia, a strong decrease in blood pressure and heart failure. It is prohibited to take Ca channel blockers within 48 hours after discontinuing the use of β-blockers.
A severe form of bradycardia can develop when the drug is combined with clonidine, reserpine, and guanfacine.

Storage conditions
Tenoric should be kept in a dark, dry place, out of the reach of small children. The drug should be stored at a temperature of 20-25°C.
Shelf life
Tenoric can be used within 36 months from the date of manufacture of the medicinal product.
Application for children
Use by people under 18 years of age is prohibited.
