Tenaxum is a selective imidazole-terminal agonist. It belongs to the group of antihypertensive drugs.

It is used to treat primary hypertension.

Release form

The release occurs in tablet form, 15 pieces inside a blister plate. The pack contains 2 such plates.

Pharmacodynamics

Tenaxum is an antihypertensive drug from the oxazoline category. It is selectively synthesized with I1-imidazole endings inside the central and peripheral (mainly renal) vasomotor nerve centers. Synthesis of the substance rilmenidine with imidazole endings slows down the sympathomimetic activity of the nerve centers, which have a peripheral and cortical form, and also causes a decrease in blood pressure.

The medication leads to a decrease in diastolic and systolic blood pressure (the degree of decrease in the indicators depends on the size of the drug portion), exerting a therapeutic effect both in the horizontal and vertical positions of the patient.

When administered in medicinal doses, the duration of action is 24 hours.

Pharmacokinetics

Suction.

The plasma Cmax level (3.5 ng/ml) is observed 1.5-2 hours after a single dose of 1 mg of the drug. Bioavailability indicators are 100%. The drug is not subject to first liver passage.

The concentration of the drug is stable and has no individual deviations. Taking it with food does not affect bioavailability indicators. The degree of absorption when used in medicinal portions does not change.

Distribution processes within tissues.

Synthesis with blood plasma protein is less than 10%. Distribution volume indicators are 5 l/kg.

Metabolic processes.

The drug is extremely poorly metabolized. Only a small part of its decay products is noted in the urine - they are a consequence of oxidation or hydrolysis of the oxazoline ring. These metabolic products do not affect the α2-endings.

Excretion.

The drug is mainly excreted through the kidneys: 65% of the consumed dose is excreted unchanged in the urine. The renal clearance rate is 2/3 of the total clearance rate.

The half-life of the drug is 8 hours. This indicator is not tied to the size of the portion consumed and its repeated intake. The medicinal effect is longer lasting, a significant hypotensive effect is maintained for 24 hours from the moment of drug use in people with elevated blood pressure who take it in a daily dose of 1 mg.

The phase of stable equilibrium parameters within the blood plasma is observed after 3 days and remains at the same level for 10 days.

Dosing and administration

It is required to take 1 mg of the drug orally per day. If the desired result is not achieved after 1 month of taking the drug, it is allowed to increase the daily dose to 2 mg, which are taken in 2 doses. The therapy is carried out in a long cycle.

Use Tenaxuma during pregnancy

It is prohibited to use Tenaxum during pregnancy.

If it is necessary to take the drug during the lactation period, it is necessary to stop breastfeeding, because rilmenidine can be excreted in breast milk.

During experimental tests it was revealed that the component rilmenidine does not have embryotoxic or teratogenic properties.

Contraindications

Main contraindications:

  • a state of severe depression;
  • chronic renal failure (CC values are <15 ml/minute);
  • presence of hypersensitivity to rilmenidine.

Side effects Tenaxuma

Taking the drug may lead to the occurrence of certain side effects:

  • dysfunction of the cardiovascular system: development of palpitations. At the same time, orthostatic collapse, cooling of the extremities or hot flashes are occasionally observed;
  • disorders of the central nervous system: a feeling of weakness due to physical exertion, a feeling of drowsiness or insomnia. In addition, convulsions, a feeling of anxiety or depression may occasionally appear;
  • problems affecting digestive activity: dry mouth, pain in the epigastric region, and diarrhea. Constipation or nausea are observed sporadically;
  • dermatological symptoms: swelling or rash. Itching develops occasionally;
  • others: sexual dysfunction is observed sporadically.

Overdose

In case of poisoning with the drug, symptoms such as attention deficit disorder or a marked decrease in blood pressure are observed.

To eliminate the disorders, it is necessary to wash the patient's stomach, and also prescribe sympathomimetics (if necessary). Excretion of the substance using dialysis procedures is very insignificant.

Interactions with other drugs

After combined use with diuretics, vasodilators, and antihistamines, the hypotensive effect of rilmenidine increases.

The combination of Tenaxum with tricyclics leads to a weakening of the hypotensive properties of the element rilmenidine.

Tenaxum is kept in standard drug conditions, in a place out of reach of children.

Tenaxum is approved for use within 24 months from the date of release of the therapeutic agent.

Application for children

There is no data on the medicinal efficacy and safety when prescribed to children, which is why it cannot be used for this age group.
