The preparation of Cyronem (manufacturer - Simpex Pharma Pvt. Ltd, India) is an antibiotic of a group of carbapenems (beta-lactams). Other trade names of antimicrobial agents of this pharmacological group are: Meronem, Meropenem, Ipinem, Doriprex, Doripenem, Propinem, and others.

Indications Saeronym

Cyronem (active substance - meropenem) is used in the treatment of severe forms of inflammatory diseases caused by microbes resistant to the effects of other antibiotics (penicillin, ampicillin, etc.). These are diseases such as pneumonia (including complicated nosocomial infection); cystic fibrosis and chronic bronchial and pulmonary infections; urological infectious pathologies ( pyelonephritis, including purulent-septic forms); severe forms of infection of the abdominal cavity and small pelvis ( appendicitis, peritonitis ); gynecological infections (including postpartum); polymicrobial infections of soft tissues and skin ( face, impetigo ); meningitis; bacterial contamination of blood ( sepsis and septicemia).

Release form

Form release of the drug: powder in vials (500 mg and 1000 mg) for the preparation of a solution for intravenous injection.

Pharmacodynamics

The pharmacological action of the saironem is based on the ability of the active substance of the preparation to rapidly penetrate the membranes of cells of gram-positive and gram-negative aerobic bacteria (pneumococci, gonococcus, meningococcus, staphylococcus) and anaerobic bacteria (Aeromonas caviae, Klebsiella ozaenae, Enterobacter spp, Salmonella spp, Serratia marcescens, Shigella sonnei and a lot others).

As a result of increased antimicrobial activity of the drug, the process of biosynthesis in the cells of bacteria is inhibited by binding of specific proteins of their cell wall (beta-lactams). This enzyme makes bacteria resistant to most antibiotics, but Cyroonum neutralizes beta-lactamase, which leads to the destruction (lysis) of cells of pathogenic microorganisms.

This is due to the pronounced antimicrobial effect of Sironem against the causative agents of many diseases. However, bacteria such as Xanthomonas maltophilia, Enterococcus faecium, as well as strains of Staphylococcus spp resistant to antibiotic methicillin, show resistance to the preparation of Cyron.

Pharmacokinetics

The drug is administered intravenously or intramuscularly and penetrates well into most tissues and body fluids (blood plasma, cerebrospinal fluid in patients with bacterial meningitis, etc.). Metabolized with the formation of one microbiologically inactive metabolite. When injected into a vein, the half-life of the drug (T1 / 2) in adults (in the absence of kidney pathology) is one hour, with intramuscular injection - an hour and a half.

Approximately 2% of the dose binds to plasma proteins, about 70% is unchanged in urine within 12 hours. In lactating women, the drug is excreted in breast milk.

Dosing and administration

For adults, a single dose of intravenous administration is from 500 mg to 2 g. The frequency of administration of the drug, as well as the duration of treatment are determined depending on the indications and severity of the disease. Dosage for children weighing up to 50 kg is 10-12 mg per kilogram, with a body weight of over 50 kg, doses similar to adults. The drug is administered every 8 hours.

When administered intramuscularly, the adult dose is 500 mg every 8 hours, the elderly patients every 12 hours.

Use Saeronym during pregnancy

The use of Cyroneme during pregnancy and lactation is allowed only after comparing the expected therapeutic benefit to the mother and the possible risk to the fetus or child. The use of Cyronem for the treatment of children under 3 months is not recommended.

Contraindications

Contraindications to the use of Cyronem are the nature of "special instructions", which include:

  • hypersensitivity to penicillins and antibiotics of carbapenems (beta-lactams). In this case, there is a possibility of complication of antibiotic treatment in the form of specific dysbacteriosis and inflammatory bowel disease (pseudomembranous enterocolitis);
  • presence of liver diseases and lower respiratory tract diseases caused by Pseudomonas aeruginosa;
  • at liver pathologies it is necessary to control the level of bilirubin and liver transaminases (ALT and AST).

Side effects Saeronym

Side effects of the drug may occur in the form of a headache; unpleasant sensations such as burning, crawling, numbness (paresthesia);

Pain in the epigastric region; nausea, vomiting, diarrhea; skin itching, rash, hives.

It is possible to increase (reversible) the levels of bilirubin, transaminases, alkaline phosphase (AP) and lactate dehydrogenase (LDH) in the blood; decrease in hemoglobin level; increased urea concentration in plasma.

Also, there may be a violation of kidney function (hypercreatininaemia), hematuria and vaginal candidiasis. Local reactions - inflammation, phlebitis, thrombophlebitis, tenderness, swelling at the injection site.

Overdose

Cases of Syronem overdose have not been noted in clinical practice.

Interactions with other drugs

As a result of the study of the interaction of Cyronem with other drugs, it was concluded: do not recommend the use of Syronem in combination treatment with uricolytic drugs (probenecid, benefi, Santuril). These antidotal drugs can delay the excretion of other drugs by the kidneys, increasing their concentrations in the blood plasma.
