Indications Ranostopa

The ointment is indicated as a preventive measure against infections in case of minor abrasions or cuts, as well as burns and minor surgical operations. In addition, it is used to treat skin infections (most often bacterial or fungal), as well as trophic ulcers and bedsores, in which infection develops.

Release form

It is produced in ointment form. The tube volume can be 20, 40 or 100 g. A separate package contains 1 tube of ointment.

Pharmacodynamics

The active component of the medicine is povidone-iodine. This is a complex compound of iodine with the polymer E1201, which releases iodine for some time after the skin is treated with the ointment. Elemental iodine has a bactericidal effect and has a fairly wide range of antimicrobial action against various viruses and bacteria, as well as fungi and protozoa.

The medicine works according to the following scheme: free iodine has a rapid bactericidal effect on the body, and the polymer acts as a depot for this component.

After contact with mucous membranes and skin, the polymer releases a large amount of iodine.

Iodine interacts with hydroxyl and oxidative-sulfide particles of amino acids, which are components of enzymes and protein structures of microbes. It helps to destroy or suppress the activity of these proteins. Many microbes disappear under the influence in vitro very quickly (less than 1 minute is required), the main destructive effect occurs in the initial 15-30 seconds. In the process, iodine discoloration occurs, so a change in the shades of brown saturation is considered an indicator of the effectiveness of the substance.

The active component of the drug is able to affect gram-negative and gram-positive microbes (bactericidal effect), as well as fungi (fungicidal effect), viruses (virucidal effect), fungal spores (sporicidal effect) and individual simple microbes (protozoal effect). The mechanism of action of Ranostop does not cause the development of resistance (this includes the secondary form of resistance in case of prolonged use of the ointment).

The ointment can be easily washed off with water because it dissolves well in it.

Long-term treatment of wounds or severe burns on large areas of skin/mucous membranes with the drug may cause absorption of a significant amount of iodine. Often, after long-term use of the drug, the level of iodine in the blood increases. This indicator returns to its original value 1-2 weeks after the last application of the ointment.

Pharmacokinetics

The absorption properties of povidone iodine are similar to those of regular iodine.

The volume of distribution is approximately 38% of the weight, and the half-life (with intravaginal administration) is approximately 2 days. The typical total plasma iodine level is approximately 3.8-6 mcg/dL, and the inorganic form is 0.01-0.5 mcg/dL.

Excretion is mainly carried out by the kidneys. Clearance is 15-60 ml/minute (the exact figure depends on the plasma iodine level, as well as the level of CC (the norm is 100-300 mcg of iodine in 1 g of creatinine)).

Dosing and administration

The medicine is applied to the skin locally.

When treating infectious processes, it is necessary to treat the affected areas 1-2 times a day. For prevention - treatment 1-2 times a week, as long as there is a need.

The product should be applied to dry and cleansed skin, in a thin layer. After the procedure, a bandage may be applied to the affected area of skin.

Use Ranostopa during pregnancy

The component povidone-iodine has no teratogenic properties, but its use during pregnancy after the 2nd month, as well as during lactation, is prohibited (in the latter case, if it is necessary to use the drug, it is necessary to stop breastfeeding for the duration of treatment). Iodine can penetrate the placenta, as well as into breast milk.

Contraindications

Contraindications are the following problems:

  • high sensitivity to iodine or other elements of the drug;
  • the presence of an adenoma, thyrotoxicosis or thyroid disorders (for example, colloid goiter (nodular type) or diffuse goiter, as well as autoimmune thyroiditis);
  • Duhring's dermatitis (herpetiform type);
  • the time period before a course of treatment using radioactive iodine (or after its completion);
  • kidney failure.

Newborns and infants under 1 year of age are allowed to use the medicine only if there are strict indications.

Side effects Ranostopa

As a result of using the ointment, the following side effects may develop:

Indications for tests: development of metabolic acidosis, changes in serum electrolyte levels (development of hypernatremia), and also osmolarity;

Urinary system and kidneys: acute renal failure and functional renal disorder;

Subcutaneous layers and skin: local manifestations of increased sensitivity (contact dermatitis, which results in the formation of a small red bullous rash similar to psoriasis), as well as allergies in the form of redness, itching and rashes, as well as Quincke's edema;

Endocrine reactions: development of thyrotoxicosis. In case of prolonged use of povidone-iodine, the level of iodine in the body may increase.

There have been reports of iodine-induced thyrotoxicosis (with prolonged use of the ointment). This problem has often been observed in people who already have thyroid disease.

General negative manifestations in acute form were noted sporadically – decreased blood pressure and difficulty breathing (anaphylactic reactions).

Overdose

In acute iodine poisoning, a person experiences the following symptoms: metallic taste, increased salivation, pain or burning sensation in the throat/oral cavity. In addition, swelling of the eyes and irritation occur. Gastrointestinal disorders, skin reactions, functional renal impairment with anuria and circulatory problems develop. Laryngeal edema is possible, developing into secondary asphyxia, pulmonary edema, development of hypernatremia and metabolic acidosis.

Overdose can be treated with supportive care and symptomatic relief. Particular attention should be paid to kidney and thyroid function, as well as electrolyte balance.

Interactions with other drugs

The active substance of the drug is most effective against pathogenic microbes at pH values of 2-7. When combined with proteins and other organic structures, the activity of the drug is weakened.

Combined use with enzymatic wound-healing ointments weakens the effectiveness of both drugs.

It is possible that interactions with the active component of Ranostop may develop when combined with drugs containing silver and mercury, as well as taurolidine and hydrogen peroxide, which is why such combinations are prohibited.

Storage conditions

The medicine must be kept in a place inaccessible to children. Temperature indicators in the storage place are a maximum of 25 o C.

Shelf life

Ranostop is permitted to be used for 2 years from the date of release of the medicinal ointment.
