Release form

The medicine is available in the form of white tablets or with a yellowish tinge. Tablets have a flat-cylindrical shape, with a dividing strip. On the one hand, the trademark of the enterprise that produces Pyatirchatka IC can be printed.

Pharmacodynamics

Pyatirchatka IC at the reception has a combined effect: reduces temperature, anesthetizes, eliminates inflammation.

Included in the Pyatirchatka IC analgin (metamizol sodium) anesthetizes and reduces fever, relaxes the smooth muscles of the urinary, biliary tract, uterus. The therapeutic effect can be observed after 20-40 minutes, the maximum effect of Pyatirchatka IC manifests itself in two hours. Paracetamol has a slight analgesic effect and relieves fever well. Caffeine acts excitantly on certain centers of the medulla oblongata, promotes the penetration of analgesics through the blood-brain barrier, prevents collapse, reduces the sedative effect of other components of the Fivefold IC.

Phenobarbital acts soothingly, removes spasms, activates the action of analgesics included in the drug, has a muscle relaxant effect (decreased muscle tone).

Codeine - a narcotic analgesic, reduces the excitability of the cough center, anesthetizes. Pyatirchatka IC in small doses does not depress the respiratory center, does not reduce the secretion of bronchi and does not disrupt the work of the ciliated epithelium. It enhances the action of sedatives, hypnotics and pain medications. With frequent unreasonable use of drugs, which include codeine there is a disruption of the gastrointestinal tract.

Pharmacokinetics

Analgin is absorbed in the gastrointestinal tract, cleavage occurs in the intestine with the formation of an active metabolite, which binds to blood cells by 50-60%, followed by cleavage in the liver, finally excreted by the kidneys.

Also quickly occurs by absorption of Paracetamol in the gastrointestinal tract, then the component binds to the blood cells. In the liver, cleavage occurs with the formation of glucuronide and paracetamol sulfate. It is mainly excreted by the kidneys.

Absorption of caffeine occurs in the intestine, then the component is split in the liver, from the body it is excreted by the kidneys.

Phenobarbital is absorbed as well as other components in the gastrointestinal tract, but slowly enough. It is split in the liver. The half-life of the organism is 3-4 days. It is excreted by the kidneys (25-50% unchanged), the substance crosses the placental barrier well.

Codeine, due to its similarity with fats, crosses the blood-brain barrier well, accumulates mainly in adipose tissue, a small amount can be observed in some internal organs (lungs, spleen, kidneys, liver). It is mainly excreted by the kidneys in the form of metabolites, a small amount of substance is excreted with bile.

Renal failure can lead to the accumulation of active metabolites, which will affect the duration of codeine action in the body.

Dosing and administration

Pyatirchatka IC is taken orally. Adults and children from 12 years should take one tablet 1 to 2 times a day. At one time you can not take more than two tablets, a day you can drink no more than six tablets.

Duration of treatment is five days. The recommended period of treatment can be extended only by the appointment of a specialist and under his strict control.

Use Pyatirchatka IC during pregnancy

Pyatirchatka IC is not recommended during pregnancy or lactation. Because of the phenobarbital that is part of the phenobarbital, it is not recommended for pregnant women to receive Pyatirchatka IC because it easily penetrates the placenta and can disrupt blood clotting in a newborn baby; in addition, Pyatirchatka IC can provoke postpartum haemorrhage.

Contraindications

Pyatirchatka IC is contraindicated in case of increased susceptibility to any component included in the preparation. Do not take medication with increased intraocular pressure, intracranial hypertension, liver or kidney dysfunction, bronchospasm, various blood disorders (anemia, leukopenia, etc.), lack of glucose-6-phosphate dehydrogenase, peptic ulcer, children under 12 years of age, women during pregnancy or lactation.

Side effects Pyatirchatka IC

Typically, the resulting adverse reactions, after taking Pyatirchatka IC have a short action and after stopping the reception pass independently. In patients with a strong susceptibility to certain components of the drug, the likelihood of developing an anaphylactic shock, which can manifest independently of the daily dose, increases.

Pyatirchatka IC can provoke the appearance of itching, skin rashes, bronchopasm, dyspnea, Quincke's edema, severe erythema form (vesicles on the oral mucosa). On the part of the digestive system, there may be a feeling of nausea, vomiting, increased activity of liver enzymes, dry mouth, constipation (or diarrhea), a total or partial loss of appetite.

Reception of the drug can provoke a malfunction in the nervous system: trembling limbs, increased irritability and fast fatigue, impaired coordination, sleep, anxiety.

Reception of the drug can trigger a decrease in blood pressure, palpitations, in some cases, anemia can develop in various forms.

Long and uncontrolled use of the drug in large doses can lead to dependence.

Overdose

Pyatirchatka IC in case of overdose provokes nausea, vomiting, rapid heartbeat, depression, various allergic manifestations, decreased heart rate, feeling weak. In case of drug overdose, it is recommended to provoke vomiting and receive activated charcoal, as well as symptomatic treatment.

Interactions with other drugs

Pyatirchatka IC when taken together with non-narcotic analgesics can provoke more pronounced toxic effects. Paracetamol can reduce the effect of medications that are broken down in the liver. Rifampicin reduces the analgesic effect of paracetamol, cimetidine reduces toxic effects and increases the analgesic effect.

Classical antidepressants, oral contraceptives, allopurinol increase the toxic effects of analgin, inducers of microsomal hepatic enzymes reduce the therapeutic effect of the drug. When taking analgin and cyclosporine, there is a significant decrease in the level of the latter in the blood.

Sleeping pills, sedatives, barbiturates, benzodiazepines, and alcohol are more active when taken concurrently with codeine (one of the components of the Five-arm IC). Also, simultaneous reception of codeine and antidepressants and monoamine oxidase inhibitors, can mutually increase the therapeutic effect of the components.

Caffeine lowers the effect of hypnotic drugs. Phenobarbital increases the rate of chemical reactions in the body of substances such as carbamazepine, estrogen, phenytoin, doxycycline, griseofulvin, quinidine. The inhibitory effect of phenobarbital is enhanced with alcohol intake, classical antidepressants, narcotic analgesics, phenothiazine.

Soothing drugs and tranquilizers increase the anesthetic effect of Pyatigar IC. When taking with alcohol increases the likelihood of impaired liver function, because there is an increase in the toxic effect on the organ. The chemical reaction in the body of phenobarbital decreases with the parallel intake of sodium valproate and valproic acid.
