Heparin Na is a direct anticoagulant, a natural antiplatelet factor of the human body.
Dexpanthenol is a derivative of pantothenic acid.

Indications Fitobena
It is used in case of injuries (including sports), muscle damage, bursitis, hematomas, tendovaginitis with tendinitis, and also in case of damage to joints or periarticular systems (not affecting the epidermis), scapulohumeral periarthritis and shoulder epicondylitis. It is also prescribed in case of neuralgia in the active phase.
Release form
The medicine is released in the form of a gel, inside tubes with a volume of 20, 40 or 100 g. There is 1 tube inside the box.
Pharmacodynamics
Dimethyl sulfoxide has an effect by inactivating hydroxyl radicals and improving metabolic processes within the inflammation zone, and in addition, reducing the speed of movement of nociceptive reactions within peripheral nerves. This component overcomes biological membranes and facilitates the resorption of other elements of the drug.
Heparin Na, by inactivating tissue biogenic amines, demonstrates moderate anti-inflammatory activity, prevents the formation of blood clots, improves microcirculation processes, activates fibrinolytic parameters of the blood and helps to heal connective tissue by suppressing the action of hyaluronidase.
Dexpanthenol inside the skin is transformed into vitamin B5, which is included in the structure of coenzyme A and is an important element of oxidative and acetylating processes. Helping to improve metabolism, it stimulates the healing of damaged tissues.
Dosing and administration
Apply the medicine to the affected areas or around them (in the case of treating abrasions) in a thin layer (for example, a 3 cm gel strip is used for an area that is similar in size to the projection of the knee joint), evenly treating the epidermis and rubbing in lightly. This process should be repeated 2-4 times a day.
You can use airtight dressings with the gel. The dressing procedure should be done after most of the substance has been absorbed into the epidermis (and the alcohol contained in the gel has evaporated) – after a few minutes.
During iontophoresis, a gel with good contact characteristics and containing effective active ingredients complements the physiotherapeutic effect of ultrasound waves.
The duration of therapy is selected individually for the patient - the doctor takes into account the medicinal effect of the gel and the intensity of the existing disease.
Use Fitobena during pregnancy
There is only limited information regarding the use of dimethyl sulfoxide in pregnant women, which is why Fitobene is prohibited for use during this period.
Dimethyl sulfoxide can be excreted in breast milk, so it is not used during breastfeeding.
Contraindications
Main contraindications:
- severe intolerance to the components of the drug;
- severe kidney/liver dysfunction;
- BA;
- severe problems with the function of the cardiovascular system (myocardial infarction, severe angina, intense systemic atherosclerosis and stroke);
- trophic ulcers affecting the lower extremities;
- open, bleeding or infected wounds;
- purpura, tendency to bleeding, hemorrhagic diathesis, hemophilia and thrombocytopenia;

Side effects Fitobena
Side effects associated with the activity of dimethyl sulfoxide: adynamia, diarrhea, dizziness, dermatitis, insomnia and bronchial spasm, as well as vomiting, headaches, nausea and signs of intolerance, including Quincke's edema, epidermal dryness and rashes. In addition, transient manifestations in the form of itching, burning and redness at the treatment site and a slight garlic odor from the mouth may be observed. Taste disturbance, sometimes developing immediately after using the gel, usually disappears after a few minutes.
Disorders caused by the effect of dexpanthenol: erythema, eczema, dermatitis of allergic or contact nature, itching, blisters and epidermal irritation, urticaria and rashes.
Problems associated with the effects of heparin: epidermal rash or swelling, signs of intolerance and hemorrhages. Sometimes small vesicles, pustules or blisters may be observed, which quickly disappear after stopping the use of Fitobene. In case of treatment of large areas of the epidermis, systemic negative signs may be observed.
Sometimes allergy symptoms develop, including Quincke's edema and urticaria.
If negative signs or other unusual symptoms appear, you should stop taking the medication and consult your doctor regarding further treatment.
Interactions with other drugs
The use of heparin may result in prolongation of PT values in individuals using oral anticoagulants.
It is forbidden to combine Fitobene with substances applied topically, for example, with medications containing hydrocortisone, anticoagulants, tetracycline and salicylic acid.
Due to the ability of dimethyl sulfoxide to potentiate the specific effects and toxicity of individual medicinal substances, it is necessary to avoid the combined use of the drug with other local medications.
Due to the presence of dimethyl sulfoxide in the drug, it is prohibited to use it together with sulindac (an NSAID), because this can provoke the development of severe toxic symptoms (polyneuropathy).
Dimethyl sulfoxide potentiates the activity of ethyl alcohol (and alcohol slows down the excretion of the drug), butadion, aspirin with insulin, quinidine, digitalis substances, antibiotics (including monomycin with streptomycin) and nitroglycerin, and in addition, sensitizes the patient's body to anesthetic substances.
Shelf life
Fitobene can be used within a 3-year period from the date of manufacture of the medicinal substance.
Application for children
It cannot be used in pediatrics because there is not enough information regarding the use of the drug in this group.
Analogues
The analogs of the drug are Alorom, Nizer, Mustard plaster with Algasan, Kapsikam and Ungapiven with Deep Freeze, and in addition to this Betalgon, Percutalzhin and Vim-1 with camphor alcohol and canned medical bile.
Reviews
Fitobene copes well with sprains and other disorders that are indicated in the indications of the drug - its effectiveness is confirmed by reviews left by many patients.
