Palixide-Richter (synonyms - Doperezil, Alzepil, Arisept, Yasnal) refers to drugs, the action of which is directed to the neurotransmitter processes of the peripheral nervous system, in particular, to the afferent nerves and cholinergic synapses.

Indications Palixide-Richter

Palixide-Richter is used in the symptomatic treatment of such neurodegenerative diseases as:

  • Alzheimer's disease (moderate severity),
  • presenilnye and senile dementia Alzheimer's type,
  • dementia of vascular (cerebrovascular) etiology,
  • Dementia in the disease of diffuse Levi bodies,
  • minor cognitive disorders (thinking, memory, speech) of the amnestic type.

Release form

Palixide-Richter is available in the form of coated tablets of 5 and 10 mg (on a blister of 20 pieces).

Pharmacodynamics

The active substance of the drug Palixide-Richter - donepezil hydrochloride - reduces the activity of the enzyme acetylcholinesterase, which affects the biosynthesis and metabolism of acetylcholine - one of the main endogenous neurotransmitters. The cleavage and neutralization of acetylcholine leads to a decrease in neurohumoral and synaptic transmission of excitation in the endings of peripheral nerves.

Donepezil hydrochloride, inhibiting acetylcholinesterase, stops the destruction of acetylcholine, which increases the level of acetylcholine in the hippocampus and cortex, activates cholinergic receptors and synapses, and as a whole - improves the transmission of excitation to the central nervous system. Thus, the progression of Alzheimer's disease and dementia slows down, and the condition of patients improves.

Pharmacokinetics

After taking Palixid-Richter, the maximum concentration of donepezil hydrochloride in the blood plasma is observed on average after 3.5 hours. Moreover, more than 90% of the drug binds to plasma proteins, and its half-life is about three days.

70% of the preparation Palixide-Richter is transformed by the liver (under the action of isoenzymes CYP3A4 and CYP2D6), 30% of the dose received is not metabolized. Metabolites and unmodified donepezil can be in the body for more than seven days, excreted in the urine (about 60%) and feces.

Dosing and administration

The drug Palixide-Richter is taken orally once a day (in the evening), regardless of food intake. The initial dose is 5 mg. In such a dosage the drug is taken within a month, after which the doctor assesses its effectiveness. Dosage can be increased to 10 mg per day (once).

The method of application of this drug requires regular monitoring of the patient's condition.

Use Palixide-Richter during pregnancy

The use of Palixide-Richter during pregnancy and during lactation has not been studied, as clinical experience of its use by pregnant and lactating women is absent. 

Contraindications

Among the contraindications to the use of this drug are: increased sensitivity to donepezil hydrochloride and age to 18 years.

The use of Palixide-Richter for the treatment of patients with heart rhythm disorders, bronchial asthma, peptic ulcer of the stomach and duodenum requires caution.

Side effects Palixide-Richter

To date, the list of side effects Palixid-Richter appear: headache, dizziness, fainting, fatigue, seizures, sleep disorders, hallucinations, agitation, aggressiveness; rash and itching of the skin; nausea, vomiting, gastrointestinal disorders; urinary incontinence.
