Oxaliplatin medak is a platinum-containing drug used in oncological diseases. Oxaliplatin was first discovered almost forty years ago by Japanese scientists, and since 1979 it has been used as an advanced chemotherapeutic agent in the uncompromising fight against cancerous tumors of the rectum and colon. Oxoliplatin medak contains platinum compounds with oxalate and complexone. The complexone in this compound is 1,2-diaminocyclohexane. Oxaliplatin medak contains lactose monohydrate, which helps the main component act. This mixture is a dirty white mass, similar to last year's snow.

Indications Oxaliplatin medac

Oxaliplatin medac is used as a chemotherapeutic agent for inoperable gastrointestinal cancers, malignant bowel lesions, or after surgical removal of the original tumor, almost always simultaneously with 5-fluoroacil and water-soluble vitamin B 9. Also, indications for the use of Oxaliplatin medac include the fight against diseases such as cytotoxicant disseminated colorectal cancer, which is characterized by the presence of many individual, not yet merged with each other, affected areas. Oxaliplatin medac is used as a chemotherapeutic agent in antitumor chemotherapy.

Release form

Oxaliplatin medac is available in the form of lyophilisate in small transparent glass vials weighing: 50, 100, and 150 mg.

Oxaliplatin medak lyophysate is a powder, usually white. Lactose monohydrate brightens the platinum component of the oncologic drug. Oxaliplatin medak is used for an infusion composition, which is prepared from the lyophysate of the drug. Lactose monohydrate is used as a substance that helps preserve the lyophysate of Oxaliplatin medak without losing its structural integrity and biological activity.

Pharmacodynamics

Oxaliplatin medak is an oncological drug used as a cytostatic, in which platinum atoms form a stable compound with a dianion and 1,2-diaminohexane. Combined use with 5-fluoracil and vitamin B9 increases the cytotoxicity of Oxaliplatin medak. Water-sodium solutions of Oxaliplatin medak disrupt cellular metabolism, suppress the synthesis of deoxyribonucleic acid. The effect of the metabolic poison Oxaliplatin medak leads to the destruction of tumor cells and, as a result, to an antitumor effect.

Pharmacokinetics

The high rate of metabolism of the drug Oxoliplatin medak leads to the fact that at the end of the administration, which lasts from 2 to 6 hours (depending on the patient's well-being) in a dose of 135 mg.m2 can no longer be accurately determined, eighty-five percent of the drug is distributed in the tissues and only fifteen percent of the administered cytostatic is present in the blood. Pt binds blood proteins, forming new albumin bridges. Oxoliplatin medak is intensively eliminated from the body with urine in the first two days. In the first hundred hours, 54% of the drug taken is excreted with urine, a very small part of the cytooxidant is excreted with feces. In case of renal failure, the rate of elimination of the cytostatic Oxoliplatin medak from the body is significantly reduced, which leads to an increase in the number and strength of undesirable side effects. With very poor renal function, the elimination of the cytostatic active substance is extremely difficult and the attending physician should review the norms and doses of the drug.

Dosing and administration

Oxaliplatin medac is administered by infusion. The infusion lasts 6-8 hours. The drug is prohibited for use in children. The infusion solution is prepared immediately before use. When administered together with 5-fluorouracil, the infusion of the cytostatic Oxaliplatin medac should be administered first. Adjuvant therapy after surgical removal of the primary colorectal cancer neoplasm is carried out at 85 mg / m 2 once every 14 days (12 cycles). For disseminated colorectal cancer, the same doses are used as for adjuvant therapy with fluorouracil. The next course of use of Oxaliplatin medac is possible only after a thorough examination of the hematopoietic system. In this case, neutrophil levels should be greater than 1500 μl and platelets more than 50,000 μl. In case of any violations, the next course should be prescribed only after the indicators are restored. In case of acute pain symptoms that last more than a week, the next dose of Oxaliplatin medac should be reduced: for adjuvant therapy by 10 mg/ m2 and for disseminated colorectal cancer by 20 mg/ m2. If sensitivity disorders persist until the next cycle, Oxaliplatin medac is discontinued.

Use Oxaliplatin medac during pregnancy

In case of any oncological diseases, pregnancy must be terminated. Oxaliplatin medak cannot be used by pregnant and nursing mothers. This is due to the fact that Oxaliplatin medak is one of the strongest metabolic poisons. If this poison gets into the milk of a nursing mother, it will pose an immediate threat to the life of the baby. Often, during pregnancy and lactation, it is necessary to refuse to use this drug. An exception may be the appointment of the attending physician, but the use of Oxaliplatin medak during pregnancy is prohibited.

Contraindications

Bone marrow always responds to radiation, chemotherapy, or biologically active agent therapy by suppressing its work. This reaction of the body is called myelosuppression and it, along with leukopenia (when the total number of peripheral blood cells decreases), is the main contraindication to the prescription of the drug Oxalapatin medak. The cytostatic is also contraindicated for patients with peripheral neuropathy, which is accompanied by impaired sensitivity of the extremities, facial muscle spasms, with diseases of the respiratory system and gastrointestinal tract. Peripheral neuropathy can progress, mainly after taking the first course of treatment. Hypersensitivity to the metabolic poison Oxalapatin medak is rare, but it also does not allow the use of this cytostatic.

Side effects Oxaliplatin medac

Chemotherapeutic use of the cytostatic Oxaliplatin medac entails a large number of side effects. First of all, symptoms from the gastrointestinal tract are observed, diarrhea, vomiting and other disorders may occur. Hematological disorders such as thrombocytopenia and neutropenia are also among the most common side effects of the drug Oxaliplatin medac.

The frequency of side effects listed here is presented in a gradation from the most frequently observed to the very rarely observed.

The human hematopoietic system is very often the main target (after cancer cells) for metabolic poisons. This is expressed in various forms of anemia, leukopenia, neutropenia, thrombocytopenia and lymphopenia,

The digestive system also very often cannot cope with the action of the cytostatic: a painful sensation in the epigastric region, vomiting, frequent bowel movements, damage to the oral mucosa, myositis, pain in the stomach, constipation - this is far from a complete list of the possible consequences of using the chemotherapy drug.

The central nervous system reacts with peripheral sensorineural neuropathy, hypoesthesia, migraine and various other disorders.

Overdose

Overdose of the drug Oxaliplatin medak usually leads to an increase in the number and strength of side effects. From visible symptoms, these are vomiting, frequent bowel movements, discomfort in the epigastric region and throat, weakness, less often - increased sensitivity and psychic disorders of various manifestations. In case of an overdose of the cytostatic Oxaliplatin medak, careful hematological monitoring is carried out, symptomatic therapy is prescribed. There is currently no substance to neutralize the effects of the metabolic poison Oxaliplatin medak.

Interactions with other drugs

Oxaliplatin medac cannot be combined with alkaline and chlorine-containing solutions. When interacting with coagulants, the risk of bleeding increases. When interacting with non-steroidal anti-inflammatory drugs, the side effects of Oxaliplatin medac may increase. When administered together with antimetabolites, it increases the cytostatic properties of both drugs. You should tell your doctor about the medications you are using, and also carefully read the annotation of these drugs.
