Omnic has an α-adrenolytic effect.
ATC classification
Active ingredients
Pharmacological group
Pharmachologic effect
Indications Omnica
It is used for the treatment of dysuric disorders developing against the background of prostate adenoma.
Release form
The drug is released in capsules, in quantities of 10, 30 or 100 pieces per pack.
Pharmacodynamics
Tamsulosin is a substance from the category of drugs that specifically block the activity of postsynaptic α1-adrenoreceptors located inside the smooth muscles of the prostate, bladder neck and prostatic part of the urethra. After blocking the activity of α1-adrenoreceptors under the influence of tamsulosin, the tone of the muscles of the prostate, bladder and prostatic part of the urethra is weakened, resulting in an improvement in urination processes. At the same time, there is a decrease in the symptoms of filling and emptying the bladder in prostate adenoma associated with detrusor hyperactivity and increased muscle tone of these organs.
The effect of tamsulosin on α1A subtype adrenoreceptors is 20 times stronger than the effect on α1B subtype adrenoreceptors located inside the smooth muscles of blood vessels. Due to such high selectivity, the use of tamsulosin does not cause a clinically significant decrease in systemic blood pressure values (both in individuals with normal blood pressure and in individuals with elevated values), which allows the drug to be prescribed to hypertensive patients.
Pharmacokinetics
The drug is absorbed quite well inside the intestine, with almost 100% bioavailability. Taking it with food slightly slows down the absorption of the drug. An equivalent degree of absorption of the drug can be achieved by taking it daily after breakfast.
Tamsulosin has linear pharmacokinetic parameters. After a single oral administration of the drug (0.4 mg), the Cmax level in plasma is observed after 6 hours. Regular daily use of 0.4 mg of the substance already on the 5th day leads to the achievement of equilibrium values, which are approximately 2/3 higher than the level observed after a single capsule intake. Protein synthesis in plasma is 99%, and the Vd values are about 0.2 l/kg.
The transformation of the drug under the influence of liver metabolism occurs quite slowly; less active metabolic products are formed. Most of the tamsulosin remains unchanged in the blood plasma.
Tests have shown that tamsulosin may weakly induce the activity of liver microsomal enzymes.
Tamsulosin, which is in an unchanged state, is excreted mostly through the kidneys together with its metabolic products. The half-life of a single oral dose (0.4 mg) of the drug after food intake is 10 hours. After multiple doses of the drug, these values reach 13 hours.
Dosing and administration
The capsules of the drug are taken orally, immediately after breakfast. The medicine should be washed down with plain water (0.1-0.2 l). 400 mcg of the substance are taken per day (contained in 1 capsule).
It is forbidden to chew the capsules, because this may increase the rate of release of the active element of the medicine from them.

Contraindications
Main contraindications:
- the presence of severe hypersensitivity to tamsulosin or other components of the drug;
- severe liver failure;
- orthostatic collapse (also present in the anamnesis).
The drug should be used with caution in people with severe kidney disease (creatinine clearance level below 10 ml/minute).
Side effects Omnica
The use of the drug may occasionally lead to the occurrence of certain side effects:
- dizziness, development of orthostatic collapse, headaches;
- diarrhea, vomiting or nausea and constipation;
- palpitations, tachycardia or asthenia;
- retrograde ejaculation;
- signs of intolerance (this includes angioedema, itching and rashes on the epidermis).

Overdose
There is no information on acute intoxication with tamsulosin. In theory, when using very large doses of Omnic, a sharp drop in blood pressure may be observed, along with compensatory tachycardia, which requires symptomatic procedures.
Blood pressure and heart rate values may return to normal after the patient is placed in a horizontal position. If this method does not produce results, drugs that increase blood volume are prescribed within the vascular bed. If necessary, vasoconstrictor drugs may be used. In addition, kidney function should be monitored during therapy. Sorbents may also be used and the gastrointestinal tract may be cleansed (gastric lavage and laxatives).
Because tamsulosin is highly protein bound, hemodialysis procedures will be ineffective.
Interactions with other drugs
The combination of the drug with nifedipine, atenolol or enalapril does not lead to the development of any therapeutic interaction.
Combination with cimetidine causes a slight increase in plasma tamsulosin levels, and in the case of taking together with furosemide, a decrease in this value is noted. With such a drug combination, the dosage of Omnic does not need to be changed, because the plasma level of its active element remains within normal limits.
Combining the drug with diclofenac or warfarin may increase the rate of elimination of its active ingredient.
Concomitant use with other α1-adrenergic receptor antagonists may result in decreased blood pressure.
Storage conditions
Omnic must be kept at a temperature between 15-25°C.

Shelf life
Omnic can be used within 4 years from the date of release of the therapeutic drug.
Analogues
The analogs of the drug are Dalfaz, Urorek, Alfirum with Alfuzosin, and also Alfater, Dalfuzin, Cornam with Setegis, and also Vesomni and Avodart.
Reviews
Omnic in most cases receives positive feedback from both patients who have taken it and medical professionals. A large number of urologists recommend that men use this drug for prostate adenoma, prostatitis and other painful conditions during which problems with urination processes are noted. In these cases, tamsulosin, which has a relaxing effect on the prostate muscles, helps eliminate the negative signs of the disease.
It should be noted, however, that there are some negative comments regarding the drug. Side effects are often mentioned, among which the most common are decreased blood pressure and retrograde ejaculation.
At the same time, it is necessary to take into account that in the process of treating prostate adenoma, Omnic is used only to eliminate the manifestations of the disease - it eliminates dysuric disorders. Therefore, for full therapy, it is necessary to use combined schemes that combine medications from different categories used for diseases of this type.
