Indications Medogrel

It is used to prevent the development of atherothrombosis in people suffering from ACS. The drug is usually used in the treatment of such disorders:

  • recent acute myocardial infarction;
  • unstable angina;
  • myocardial infarction of a subendocardial nature;
  • the occurrence of atherothrombotic complications after myocardial infarction or stroke;
  • obliterating atherosclerosis in the area of peripheral arteries.

Release form

The substance is released in tablets with a volume of 75 mg, 30 pieces per package.

Pharmacodynamics

The main active element of the drug, the substance clopidogrel, effectively inhibits platelet aggregation processes, thereby helping to irreversibly modify their ADP endings.

Inhibition of ADP-induced platelet aggregation occurs after the first daily dose of the drug, which is 75 mg.

With repeated administration of the same dosages, this effect is potentiated, acquiring stability in the interval between days 3-7 of the treatment cycle. In this phase, the degree of suppression of platelet aggregation can fluctuate, amounting to approximately 40-60%.

After 5 days from the moment of stopping therapy, platelet aggregation returns to its original values.

Pharmacokinetics

Clopidogrel is absorbed at a fairly high rate when taken in both single and multiple daily doses of 75 mg of the substance. Plasma Cmax values are noted on average after 45 minutes from the moment of drug administration.

Extensive metabolic processes take place inside the liver, involving esterases or enzymes of the hemoprotein P450.

Excretion of Medogrel occurs approximately 2 hours after taking the drug - approximately 50% is excreted by the kidneys, and another 46% is excreted with feces.

Dosing and administration

For adults with ischemic strokes, myocardial infarctions and diseases in the peripheral arteries, the use of 1 tablet (75 mg) of the drug per day is prescribed, regardless of food intake. The most suitable duration of the therapeutic cycle is selected individually, and this should be done exclusively by a doctor.

People who have previously suffered a myocardial infarction are often prescribed medication for 25 days. And after strokes, therapy usually lasts 7 days.

After performing coronary stenting, and in addition to this, in the presence of coronary syndrome, against the background of which there is no increase in the level of the ST segment, the size of the initial medicinal portion of the drug is 0.3 g. Then the patient is prescribed 75 mg (1 tablet per day). The duration of the treatment cycle can be within 3-12 months.

Medogrel must be taken daily, and always at the same time, pre-designated. If you miss a dose, you must act according to the following scheme:

  • if less than 12 hours have passed since the time of taking the medicine, you should take the tablet as soon as you remember about it;
  • if the interval exceeds the 12-hour mark, you simply need to take a new tablet at the designated time, without compensating for the missed one (thus, doubling the dose of the medication is prohibited).

Use Medogrel during pregnancy

Due to the fact that at the moment there is no information regarding the effect of the main active element of the drug (clopidogrel) on the course of pregnancy and its excretion with breast milk, it is prohibited to prescribe the drug to lactating or pregnant women.

Contraindications

Main contraindications:

  • the presence of severe sensitivity or intolerance to the active element of the drug or excipients;
  • severe liver failure;
  • bleeding of an acute nature.

Side effects Medogrel

Most often, the medication causes the appearance of such negative symptoms: bleeding under the skin, hematomas, diarrhea, nosebleeds or gastrointestinal bleeding, pain in the abdominal area and signs of dyspepsia.

Less common are the following disorders: leuko-, thrombocyto- or neutropenia, as well as eosinophilia. Bleeding may occur, affecting the visual organs, intracranial hemorrhages, headaches, ulcers in the gastrointestinal tract, dizziness, bloating, gastritis, itching or rashes, and constipation.

Some of the uncommon side effects include fever, vasculitis, serum sickness, anemia, thrombotic purpura, urticaria, hepatitis, hallucinations and confusion, as well as rashes, increased blood pressure, taste disturbances and acute liver failure.

These negative symptoms may appear due to non-compliance with the drug administration regimen or individual hypersensitivity of the patient.

Overdose

When using the medication in dosages that are many times higher than the permissible limits, the duration of bleeding increases significantly, which can lead to the development of various complications.

Interactions with other drugs

It is forbidden to combine the drug with oral anticoagulants, because this significantly increases the likelihood of increased intensity of developing bleeding.

Concomitant use with drugs that inhibit the activity of glycoprotein type GPIIb/IIIa should be carried out with great caution in people with a predisposition to bleeding due to trauma and surgical procedures.

If Medogrel is taken together with aspirin, the latter should not be taken in a dose exceeding 0.5 g twice a day.

An increased risk of bleeding may be observed when the drug is combined with heparin.

In case of taking the drug together with naproxen and other medications from the NSAID group, the frequency and number of latent bleedings inside the gastrointestinal tract may increase.

Storage conditions

Medogrel should be kept in a dark and dry place, out of reach of children. The temperature in such a room should remain within +25°C.

Shelf life

Medogrel can be used within 12 months from the date of manufacture of the drug.

The drug's analogues are Lopirel, Gridoklein, Clopidogrel with Miogrel, Noclot, Pingel, Avix with Clopidal, Pingel Neo and Aterocard with Clopidal.

Reviews

Medogrel receives many good reviews from people who have used it. Most of them noted that the drug demonstrates high efficiency as a means of preventing the occurrence of atherosclerosis or thrombosis. It is also reported that, although the drug has a fairly large list of negative manifestations, in reality they develop only sporadically.
