Isodibut is a medicine for diabetics. Let's get acquainted with its instructions: indications, side effects, dosage and the possibility of interaction with other drugs. Isodibut belongs to the pharmacological group of hormones, their analogues and antihormonal drugs. It belongs to the category of oral antidiabetic drugs. It suppresses the activity of aldose reductase, that is, an enzyme that participates in the conversion of intracellular glucose into sorbitol. The inhibitor stimulates energy processes in the brain and inhibits the sorbitol pathway of glucose metabolism.

Indications Isodibut
Isodibut is used for the treatment and prevention of complications of diabetes.
Main indications for use:
- Diabetic cataract
- Diabetic angiopia of the lower extremities
- Nephropathy
- Retinopathy
- Polyneuropathy (somatic, cerebral, encephalopathy)
The active component of the drug stops the accumulation of sorbitol in the eye lens and nerves, preventing the development of ophthalmological diseases, disorders of the tone of the nervous and vascular systems. Starts metabolic processes in the brain.
Release form
The medicine is available in tablet form, 10 tablets per blister, 3.5 blisters per package. Its chemical name is 1,3-dioxo-1H-benzo[d,e]isoquinoline-2(3H)butyric acid. According to its physical and chemical properties: white tablets with a flat surface and a yellowish tint, have a round shape with a score line and a chamfer.
One tablet contains: 500 mg isodibut, potato starch, sodium starch, lactose (200), calcium stearate and hydroxypropyl ethylcellulose (15).
Pharmacodynamics
According to pharmacodynamics data, the drug belongs to the class of aldose reductase enzyme inhibitors. The active substance reduces the activity of aldose reductase by 1.5-3 times and increases the activity of sorbitol dehydrogenase by 1.2-1.4 times. This leads to inhibition of the sorbitol pathway of glucose metabolism and stops its accumulation in vascular walls, nerves, and lenses. Such activity leads to inhibition of protein glycosylation processes.
- Reduces the amount of glycosylated proteins in the blood and cell membranes.
- Prevents tissue damage: blood vessels, nerves, lens and stops their swelling.
- Improves metabolism and memory, accelerates metabolic processes and microcirculation of the brain.
- Improves blood supply to the retina and conjunctiva, increases visual acuity.
- Reduces albuminuria and improves renal blood flow.
- Helps restore sensitivity, accelerates the healing of wound surfaces and relieves pain in the lower extremities.
Pharmacokinetics
After oral administration, the tablets are completely absorbed from the gastrointestinal tract. Pharmacokinetics indicate that the maximum concentration in the blood is reached after 3-3.5 hours. The half-life is 6-6.5 hours.
The drug does not accumulate in the body. To maintain a constant therapeutic concentration, three doses are required during the day. The active components are excreted by the kidneys with urine.
Dosing and administration
It is recommended to take Isodibut before meals, 1 tablet 3 times a day. But for all patients, the method of administration and doses are selected by the attending physician. The maximum permissible daily dose is 8 tablets. The duration of treatment is 2-3 months with a repeat course after 6 months.
If the medicine is used to prevent complications of diabetes, the tablets are taken for 2 months 2 times a year. If the patient has gastrointestinal diseases, it is better to take the medicine after meals. Isodibut does not affect concentration and the ability to work with mechanisms.
Use Isodibut during pregnancy
Isodibut is an antidiabetic drug. Its use during pregnancy is possible only with a doctor's prescription, when the expected therapeutic benefit for the mother is higher than the potential risks to the fetus.
Contraindications
Like any pharmacological agent, Isodibut has contraindications for use:
- Individual intolerance to active components
- Patients under 14 years of age
- Severe kidney disease
- Renal failure
If these restrictions are not observed, uncontrolled adverse symptoms may appear in many organs and systems.
Side effects Isodibut

Overdose
If the dosage prescribed by the doctor or the duration of the course of therapy is exceeded, signs of overdose occur. Most often, patients experience the following symptoms of hypoglycemia:
- Excessive sweating
- Rapid heartbeat
- General weakness and anxiety
- Paleness of the skin
- Impaired coordination of movement
- Neurological disorders (impaired sensitivity, vision, speech, paralysis)
- Paresthesia in the mouth area and tremors of the limbs
- Loss of consciousness
For treatment, gastric lavage is performed and a sugar solution or glucose is taken orally. In case of severe hypoglycemia with loss of consciousness, the patient is given a 40% glucose solution or glucagon intravenously, intramuscularly or subcutaneously. To restore normal health, the patient is given food rich in carbohydrates. This helps prevent the recurrence of hypoglycemia.
Interactions with other drugs
For complex treatment, interaction of Isodibut with other drugs is possible. The medicine potentiates the action of such drugs:
- Insulin
- Angiotropic drugs
- Oral hypoglycemic drugs
- Antihypertensive drugs
- Sulfur-containing compounds (Sodium thiosulfate, Unithiol)
- A-lipoic acid
When using several drugs at the same time, medical consultation and permission is required.

Storage conditions
According to storage conditions, tablets must be kept in the original packaging, in a place protected from moisture and sunlight. The recommended storage temperature should not exceed 25 °C. Failure to comply with these recommendations leads to spoilage of the medicine and loss of its medicinal properties.
Shelf life
Isodibut can be used for 36 months from the date of manufacture. After the expiration date, the medicine is disposed of and not used for treatment. The antidiabetic drug is dispensed only with a doctor's prescription.
