Pharmacodynamics of N-desmopressin spray is precisely aimed at the pathogenesis of diabetes insipidus, the main symptoms of which are excessive urine output (polyuria) and its low density (hypoisosthenuria). The cause of polyuria is a deficiency of the antidiuretic nanopeptide neurohormone vasopressin, produced in the hypothalamus. This hormone regulates the osmolarity and osmotic pressure of fluids in the body, and also stimulates the reabsorption of water in the tubular system of the renal nephrons.

The drug H-desmopressin spray contains a synthetic analogue of the endogenous hormone arginine vasopressin - the active substance desmopressin acetate (DDAVP, 1-deamino-8-arginine vasopressin). Changes were made to its structure, which greatly increased the vasopressor activity and the effect on the V2 receptors of renal tubule cells.

N-desmopressin spray promotes increased permeability of the renal tubule membrane, which leads to increased water reabsorption and, as a consequence, to a decrease in the volume of excreted urine. In addition, the osmolar concentration of urine increases and the osmolarity of blood plasma decreases, and this reduces the frequency of urination, including at night.

Pharmacokinetics

After the introduction of 10-20 mcg of spray into the nasal passages, no more than 20% of the dose is absorbed through the nasal mucosa. 20-30 minutes after the use of the drug, its active substance is detected in the blood plasma; the maximum concentration is noted after 60 minutes. The duration of the therapeutic effect is from 8 to 12 hours.

The amount of unchanged H-desmopressin spray entering the systemic bloodstream does not exceed 3-5%. The active substance of the drug does not penetrate the BBB.

The mean half-life is 2.5 hours. A small amount of desmopressin undergoes biotransformation in the liver.

Contraindications

Contraindications to the use of H-desmopressin spray include: individual hypersensitivity to desmopressin; damage to the nasal mucosa; children under 3 months of age; pathologically increased thirst (polydipsia); coronary insufficiency; moderate and severe renal failure; chronic deficiency of sodium ions in the blood plasma (hyponatremia); neurocrine disorder in the form of inadequate production of the antidiuretic hormone vasopressin.
