Pharmacodynamics

Depres is an antidepressant, a derivative of propylamine. By its chemical composition, it differs from tricyclics and four-cycle antidepressants. The effect of the drug is due to a slowdown in the process of reverse serotonin uptake by neurons inside the CNS. The active component prevents serotonin capture, but does not affect norepinephrine.

The medicine helps to improve mood, remove dysphoria, and also reduce the feeling of tension and fear. Does not have sedative properties. In case of consumption in moderate medicinal doses, it almost does not affect the work of SSS and other systems.

Pharmacokinetics

When using a single dose of drugs at a rate of 40 mg, the active component peaks within the plasma for a period of 6-8 hours (15-55 ng / ml). Eating does not change the level of bioavailability of the drug, although the absorption of the substance may be delayed. Synthesis with a plasma protein is 94.5%.

Fluoxetine undergoes rapid hepatic metabolism, which results in the formation of the substance norfluoxetine and other inactive decay products. The medicine is then excreted through the kidneys. The half-life of the active ingredient is 2-3 days, and the active decay product, norfluoxetine, is 7-9 days.

The half-life of fluoxetine with norfluoxetine is prolonged in people with liver failure. Long-term treatment in people with severe form of this pathology can cause accumulation of the substance.

Dosing and administration

For adults.

The size of the daily required dosage for depression or OCD is 20 mg, which must be taken in the morning. If there is no effect after 3-4 weeks of the course, it is allowed to increase the daily dose to 80 mg. Dosages that exceed 20 mg per day need to be divided into 2 receptions, morning and evening.

With bulimia, it is recommended to take 60 mg of medication per day. The maximum allowable daily dose is 80 mg.

Since the drug has a long half-life (2-3 days, and its active decay product - 7-9 days), its concentration inside the plasma continues to persist for several weeks.

In the elderly.

For a day it is allowed to take no more than 60 mg of Depres.

Persons with liver or kidney failure.

In the case of functional renal disorders of moderate type (glomerular filtration rates <15-50 ml / minute), as well as liver failure, it is necessary to reduce the dosage or take the medicine every other day. In particular, this applies to elderly people with concomitant pathologies that take in combination several medicines.

  • the appearance of yawning or the development of alopecia;
  • hyperhidrosis, visual disorders (mydriasis or blurring of vision), as well as vasodilation;

Interactions with other drugs

With a combination of Depression:

  • with drugs that have an overwhelming effect on the central nervous system, and in addition to ethyl alcohol, there may be significant potentiation of the above effects, and with it an increase in the likelihood of seizures;
  • with drugs that have a high level of synthesis with protein (especially with digitoxin or anticoagulants), plasma values of free drugs can increase and the risk of negative reactions may increase;
  • with lithium drugs, as well as phenytoin - may increase the value of these drugs, as well as a toxic effect;
  • with the substance tryptophan - can increase motor anxiety, and in addition to increase the severity of disorders in the gastrointestinal tract;
  • with other antidepressants - their plasma values increase;
  • with benzodiazepines - in some patients, the half-life of diazepam may be prolonged;
  • with MAOI (among them moclobemide or selegiline) - because of the increase in serotonin levels, as well as the inhibition of its re-uptake, there is a significant increase in the volume of serotonin within the synapse. As a result, serotonin intoxication develops, against which there is rigidity of muscles, hyperthermia, and in addition myoclonus with signs of imbalance in the physiological as well as mental state of the person (with possible fatal outcome).
