Indications Daqin
Dacin is indicated for malignant metastatic melanoma. The drug is also prescribed for Hodgkin's disease, progressive soft tissue sarcoma (except Kaposi's sarcoma and mesothelialoma) as a combination treatment for adult patients.
[ 1 ]
Release form
Daqing is available as a lyophilized (dried and frozen) powder, from which a solution is prepared for injection or drip.
[ 2 ]
Pharmacodynamics
Daqing suppresses cell growth not associated with the cell cycle and inhibits DNA synthesis.
The active substance of the drug - dacarbazine can be included in other mechanisms aimed at destroying cancer cells in the body. It is generally accepted that dacarbazine does not have an antitumor effect, but due to the rapid transformation in the body into 5-amino-imidazole-4-carboxamide and methyl cation, the cytostatic effect of dacarbazine is observed.
[ 3 ]
Pharmacokinetics
When administered intravenously, dacarbazine rapidly spreads throughout the tissues. It binds to blood proteins by 5%.
The initial half-life is 20 minutes, the final half-life is from half an hour to 3.5 hours. Dacarbazine is inactive until metabolized in the liver, resulting in the formation of active N-demethylated compounds.
In the liver, dacarbazine is subject to hydrooxidation and demethylation. About 20-50% of the drug is excreted unchanged in the urine.
Dosing and administration
Dacin is administered intravenously. Treatment with the drug should be carried out under the supervision of a physician.
Dacarbazine is sensitive to sunlight and all solutions containing dacarbazine should be protected from sunlight, including during administration.
For malignant melanoma, 200-250 mg/m2 is usually prescribed once a day, the duration of therapy is 5 days (the course of treatment is repeated every three weeks).
It is also permissible to administer the drug by drip at a dosage of 850 mg/m2 once every three weeks.
For Hodgkin's disease - 375 mg/ m2 every 15 days.
For soft tissue sarcoma – 250 mg/ m2 for 1-5 days, repeat the course every three weeks.
The duration of treatment is determined by the doctor individually in each case, taking into account all factors (age, susceptibility of the body, type and stage of the disease, etc.).
The drug in a dosage of up to 200 mg/ m2 is administered slowly as an intravenous injection, higher doses (up to 850 mg/m2 ) are administered using a dropper over 15–30 minutes.
The injection solution is prepared immediately before administration; when using drip systems, it is recommended to wrap them in UV-resistant foil.
Before administration, it is recommended to visually assess the state of the solution (only transparent, fully dissolved powder should be used).
Any residues remaining after preparation, as well as rejected prepared solutions, materials used in the preparation of the drug, are subject to destruction (incineration).
Use Daqin during pregnancy
Daqing is not prescribed to pregnant women.
Patients of childbearing potential should use reliable contraception throughout the course of treatment.
Contraindications
Daqing is contraindicated in cases of hypersensitivity to some components of the drug, decreased levels of leukocytes, severe forms of liver and kidney failure.
Side effects Daqin
Daqing can cause anemia, decreased levels of platelets, leukocytes and other blood elements in the blood, anaphylactic shock, headaches, blurred vision, convulsions, numbness of the facial nerve, flushing of the face, loss of appetite, vomiting, upset stool, liver dysfunction (including necrosis), renal failure, hair loss, darkening of the skin, increased sensitivity to ultraviolet light, urticaria, flu-like syndrome, inflammation at the injection site.
Overdose
In case of overdose, Dacin suppresses hematopoietic function, which can ultimately lead to bone marrow aplasia.
A decrease in the level of platelets and leukocytes in the blood may also occur.

Interactions with other drugs
Daqing, when used simultaneously with radiotherapy and other cytostatic drugs that negatively affect the bone marrow, exhibits a stronger myelotoxic effect.
When prescribing drugs, it should be taken into account that Dacin is broken down in the liver by P450 enzymes.
With the simultaneous administration of methoxypsoralen, increased sensitivity to ultraviolet radiation is possible.
It is contraindicated to use the yellow fever vaccine simultaneously with Daqin; it is not recommended to use phenytoin.
Vaccination with vaccines containing weakened live microorganisms increases the risk of developing a systemic disease, especially in people with weakened immunity.
Immunosuppressants should be used with caution, as they increase the risk of lymphatic tissue proliferation.

Storage conditions
Daqing should be stored in a place protected from sunlight and moisture. The storage temperature should not exceed 25°C. The drug is stored in a closed place, inaccessible to children.
The injection solution prepared under aseptic conditions can be stored for no more than 24 hours at a temperature of 2 to 8°C.
Shelf life
Daqing is suitable for three years from the date of manufacture, provided that the original packaging is preserved and stored properly.
