Drops from colic used in gastrointestinal problems belong to the pharmacological group of carminative (carminative) agents, which facilitate the liberation of the intestine from gases, the accumulation of which is accompanied by paroxysmal painful spasms. Some manufacturers attribute them to agents that affect the digestive system and are used in functional intestinal disorders (ATS code - A03A) and belong to the group of silicones (ATS code - A03A X13).

It should be borne in mind that these funds are intended for those with intestinal colic. In other varieties of colic - rectal, appendicular, renal, hepatic or pancreatic - attacks of acute pain are removed by other drugs, that is, in such cases, drops from intestinal colic are not used.

Indications for use of a drop of colic

In general, drops from colic are used as a symptomatic means to reduce the excess content of intestinal gases, which happens with excessive gasification (flatulence) of any etiology (see - Causes of flatulence ); disorders of digestive processes (dyspepsia); due to increased ingestion of air in the process of food intake (aerophagy); in the presence of gastrocardial syndrome.

Gastroenterologists prescribe a colonic drop before performing instrumental and instrumental diagnostics, in particular X-rays, ultrasound and endoscopy, and before surgery on the abdominal and pelvic organs.

In addition, indications for the use of colic drops include poisoning with surfactants (synthetic detergents) that are part of detergents and other household chemicals.

Drops from colic for newborns (aged between two weeks and four months) are used in infant colic, which has a code for ICD-10 - R10.4 (symptoms and signs related to the digestive system and abdominal cavity). Despite the prevalence of this condition (which is noted in every fifth baby of the first three months of life), its pathogenesis remains unclear. However, the most common version of the cause of colic in infants connects them with the accumulation of gases in the digestive tract. That is why pediatricians recommend the use of drops from colic for newborns.

Though, probably, the given syndrome has other etiology (on which definition pediatricians of the whole world work). And, maybe, it is connected with the fact that the process of adaptation of the digestive system of the newborn, including the enzymatic one, which occurs in the small intestine, is in progress. After feeding the fetus was carried out through the placenta, bypassing the gastrointestinal tract, and after the birth of the child and the way of supply of nutrients, and their assimilation radically changed.

Pharmacodynamics

Pharmacodynamics of Espumizan L, as well as of all its synonyms, including the means for newborns, Bobotik, is provided by the active substance Simethicone (Simethicone). From the chemical point of view, this polydimethylsiloxane (dimethylpolysiloxane) is a silicone in liquid form, more precisely, modified by the introduction of methylsilyl groups, silica (silica).

This substance, getting into the digestive tract when taking drops, reduces the strength of the surface tension of the gas bubbles, and they burst. Released oxygen absorbs the intestinal mucosa, and the remaining gases (methane, hydrogen sulphide, indole, skatole, mercaptan) freely leave the intestine by flutulenia.

By the way, amorphous silicon dioxide in the form of antifoam additive Е551 is used against the caking of powdered food products and the formation of lumps in them.

The pharmacological action of the Cuplaton drop is based on their active component dimethicone (polysilane), close to the already described simethicone.

The composition of the combined drops from colic for newborns Karminativum Bebinos includes alcohol (!) Extracts of fennel, coriander and chamomile flowers. If fennel provides a carminative effect, then chamomile and coriander act as antispasmodics.

But Finnish drops from colic Rela drops (Rela Colic drops) contain lactobacillus bacteria Lactobacillus Reuteri and are a probiotic. The use of probiotics, that is, bacteria present in the gastrointestinal tract, is a promising solution to the problem of infant colic. And this is shown by randomized controlled trials (the latter was conducted in 2014 by Finnish researchers from the University of Turku).

Pharmacokinetics

Entering into drops Espumizan L, Bobotik simethicone, as well as dimethicone in the Cuplaton drop are pharmacologically and physiologically inert substances and act exclusively physically, not participating in chemical reactions. At a concentration of 0.1 mg / ml, simethicone quenches the foam in the lumen of the intestine within 3-6 seconds after taking the drug, and its duration is about 24 hours.

After oral administration of a drop containing simethicone or dimethicone, no absorption or biotransformation occurs. After passing through the gastrointestinal tract, these substances with feces are removed from the body.

According to official instructions, the pharmacokinetics of Carminativum Bebynos and Rela drops have not been studied.

To date, the list of funds in this group includes such names as a drop of colic, such as Espumizan L, produced by the German company Berlin-Chemie AG, and other trade names (used by different manufacturers) with the same active substance: Simethicone, Simikol, Infakol, Disflatil , Sab Simplex, Phazyme, Flatulex, Mylicon.
